Inaddition, some treatments will be inspected while work is in progress to ensure that the Contractor isstrictly adhering to label requirements such as space between swaths, vehicle speed and direction oftravel, etc. Work quality will be considered unsatisfactory if pretreatment counts are not reduced to therequired level. In most all instances where the quality of work is considered unsatisfactory, timely servicewill also be considered unsatisfactory. <strong>Pest</strong> management records <strong>for</strong> each service will be inspected at theend of the month when the invoice is received; spot checks will be made during the month as required toverify records are routinely being completed within 24 hours of per<strong>for</strong>mance.a. Evaluation Work Sheets. Grades of either satisfactory (S) or unsatisfactory (U) will be recordedon the attached EVALUATION WORK SHEET <strong>for</strong> each work requirement. A brief description of anynoted defects will be provided and retreatment in<strong>for</strong>mation will be recorded, if appropriate. Anyobserved violations of regulations or contract standards, such as pesticides not being applied per labeldirections, inappropriate weather conditions, etc., should be recorded in the remarks column. Providecopies of all negative EVALUATION WORK SHEETs to the Contractor.b. Call Backs. Retreatments <strong>for</strong> control failures will almost always be required and must becompleted by the Contractor within 24 hours of notification, weather permitting. Each site retreated willbe inspected by the QAE to ensure the proper level of control has been achieved. Appropriatein<strong>for</strong>mation will be recorded on the EVALUATION WORK SHEET.9. Analysis of Results. At the end of the month, the QAE will summarize the overall grades assigned <strong>for</strong>all the delivery orders completed during the month, then calculate observed defect rates (ODRs) <strong>for</strong> eachof the work requirements using the following <strong>for</strong>mula:ODR = Number of unsatisfactory overall grades x 100QA-46
