J.11 APPROVAL OF A PROPOSED CHANGE ORDER (NO. FIVE)
TO THE CONTRACT FOR THE CONSTRUCTION AT
WELLINGTON ELEMENTARY SCHOOL
(THE NEW ELEMENTARY SCHOOL AT KEITHSHIRE WAY)

BACKGROUND AND RATIONALE: The construction of a new elementary school to be located on 14.07
acres (donated by NDC) at 3280 Keithshire Way is listed as priority 1a.1 on FCPS's District Facility Plan.
GBBN Architects has produced Construction Documents (plans, specifications, bidding requirements,
etc.) describing the construction of a new school totaling approximately 72,000 SF to serve 650 students
based upon FCPS elementary school program standards. Bids were received on March 18, 2010 and
construction began in April, 2010. The Department of Physical Support Services, the design consultants
and the local building code officials have reviewed the progress of the work. Changes to the original plans
and specifications have been necessary in order to satisfy the requirements described below. Approval of
these changes allows the work to be completed.
COST: The description and cost of these items are summarized as follows:
To improve original plans and specs:
Delete one Solatube at Health Clinic and credit to one
DPO (Shaffner Heaney) ; deduct:
Provide labor, materials and equipment to add spray
insulation to provide weather-tight seal at top of
exterior walls in Library and Gym; add:
Provide labor, materials and equipment to provide
acoustical insulation to reduce noise from vacuum
pump in Janitor’s Closet into adjacent classroom;
add:
Provide labor, materials and equipment to provide
five stainless steel corner guards due to sharp cut
edges of the concrete block; add:
Provide labor, materials and equipment to provide
one additional steel handrail to Mezzanine stair for
safety; add:
To comply with building code:
Provide labor, materials and equipment to provide a
storage tank with a booster pump for rain water
collection system as required by Ky. Division of
Plumbing: add:
To resolve unforeseen conflict:
Provide labor, materials and equipment to provide
condensate pumps to drain VRF units as gravity
draining will not work; add:
Provide labor, materials and equipment to provide the
difference between the water meter allowance and
the Kentucky American Water actual charge; add:
