Priority<br />

1. General<br />

1.1. System supports a function where the contents of the paper record as def<strong>in</strong>ed by<br />

H<br />

st<strong>and</strong>ardized MOH <strong>for</strong>ms can be entered <strong>for</strong> <strong>in</strong>clusion <strong>in</strong> the EMR.<br />

1.2. System supports a total paperless function us<strong>in</strong>g a po<strong>in</strong>t of care system. L<br />

1.3. System captures data that can be used <strong>for</strong> <strong>in</strong>dividual patient care as well as <strong>for</strong> program H<br />

monitor<strong>in</strong>g <strong>and</strong> evaluation.<br />

1.4. System shall associate key identifier <strong>in</strong><strong>for</strong>mation (e.g., system ID, medical record number) H<br />

with each patient record.<br />

1.5. System allows <strong>for</strong> summarized <strong>in</strong><strong>for</strong>mation <strong>in</strong> different parts of the system to be sorted H<br />

<strong>and</strong> filtered by date or date ranges <strong>and</strong> chronology.<br />

2. Demographics/ Patient Identification<br />

2.1. System supports the generation <strong>and</strong> use of a unique identifier <strong>and</strong> allows <strong>for</strong> the storage H<br />

<strong>and</strong> use of more than one identifier <strong>for</strong> each patient’s record.<br />

2.2. System captures <strong>and</strong> ma<strong>in</strong>ta<strong>in</strong>s demographic <strong>in</strong><strong>for</strong>mation. At m<strong>in</strong>imum, the system should H<br />

capture the follow<strong>in</strong>g demographic <strong>in</strong><strong>for</strong>mation:<br />

2.2.1. Patient names: surname, first name <strong>and</strong> other name. H<br />

2.2.2. Other vital <strong>in</strong><strong>for</strong>mation <strong>in</strong>cludes: sex, date of birth, place of birth, patient’s physical H<br />

address, patient’s telephone contact <strong>and</strong> next of k<strong>in</strong>.<br />

2.2.3. Additional <strong>in</strong><strong>for</strong>mation may <strong>in</strong>clude: Biometric identifiers L<br />

2.3. System displays multiple types of patient identify<strong>in</strong>g <strong>in</strong><strong>for</strong>mation at each <strong>in</strong>teraction with H<br />

the patient record to facilitate accurate patient identification.<br />
