System Costs, Ownership <strong>and</strong> Susta<strong>in</strong>ability<br />

Requirements<br />

1. Health Managers (such as the DHMT) should be <strong>in</strong>volved <strong>in</strong> EMR system<br />

implementations to promote ownership.<br />

2. EMR systems should be developed <strong>in</strong> a manner as to m<strong>in</strong>imize the Total Cost of<br />

Ownership (TCO) while meet<strong>in</strong>g m<strong>in</strong>imum functional <strong>and</strong> operational st<strong>and</strong>ards.<br />

3. EMR system implementation should adopt an open source software where possible,<br />

to reduce on the costs of licens<strong>in</strong>g <strong>and</strong> encourage the EMR community to contribute<br />

towards system development.<br />

4. EMR system implementers should consider the resource constra<strong>in</strong>ed sett<strong>in</strong>g <strong>and</strong><br />

m<strong>in</strong>imize the resource requirements such as additional commodities that cannot be<br />

susta<strong>in</strong>ed by public health facilities.<br />

Total cost of ownership (TCO) is a f<strong>in</strong>ancial estimate of both the monetary impact <strong>and</strong> human<br />

resource impact of acquir<strong>in</strong>g, deploy<strong>in</strong>g, <strong>and</strong> retir<strong>in</strong>g an <strong>in</strong><strong>for</strong>mation technology system over the life<br />

cycle of the product. It is comprised of a number of factors that can be grouped with<strong>in</strong> three ma<strong>in</strong><br />

categories – acquisition (one time) expenses, operational (ongo<strong>in</strong>g) expenses, <strong>and</strong> long term<br />

expenses. The overall goal is to select technology that m<strong>in</strong>imizes TCO while meet<strong>in</strong>g m<strong>in</strong>imum<br />

functional <strong>and</strong> operational st<strong>and</strong>ards.<br />

When evaluat<strong>in</strong>g the total cost of ownership (TCO), the follow<strong>in</strong>g factors are to be considered:<br />

1. Acquisition Expenses<br />

a. Software licens<strong>in</strong>g<br />

b. Hardware (server, client workstations)<br />

c. Infrastructure (network<strong>in</strong>g hardware <strong>and</strong> software)<br />

d. Technical support <strong>for</strong> <strong>in</strong>stallation <strong>and</strong> configuration costs<br />

2. Operational (Ongo<strong>in</strong>g) expenses<br />

a. Hardware support<br />
