Data Transmission via HL7 <strong>and</strong> SDMX Messag<strong>in</strong>g<br />

1. EMR systems are required to have the capability to transmit <strong>and</strong> receive a def<strong>in</strong>ed<br />

m<strong>in</strong>imum set of patient data via st<strong>and</strong>ardized HL7 messag<strong>in</strong>g.<br />

2. EMR systems are required to have the capability to transmit aggregate data to the DHIS<br />

via st<strong>and</strong>ardized SDMX messag<strong>in</strong>g.<br />

Data Format <strong>St<strong>and</strong>ards</strong><br />

With the exchange of <strong>in</strong><strong>for</strong>mation, data st<strong>and</strong>ards have been developed to ensure consistency of<br />

both structure <strong>and</strong> mean<strong>in</strong>g of data between <strong>in</strong><strong>for</strong>mation systems.<br />

St<strong>and</strong>ard <strong>for</strong>mats require agreement both on <strong>for</strong>mat (syntax) <strong>and</strong> mean<strong>in</strong>g (semantics). Format is<br />

the order <strong>and</strong> structure of specific data fields, while mean<strong>in</strong>g is expressed through the choice of<br />

cod<strong>in</strong>g schemes, rules, <strong>and</strong> other constra<strong>in</strong>ts on content.<br />

St<strong>and</strong>ard <strong>for</strong>mats require agreement both on <strong>for</strong>mat (syntax) <strong>and</strong> mean<strong>in</strong>g (semantics). For<br />

transmission of patient-level data, the most widely implemented messag<strong>in</strong>g st<strong>and</strong>ard <strong>in</strong> use <strong>in</strong> the<br />

public health sector is recommended. The follow<strong>in</strong>g <strong>in</strong>ternational protocols are highly<br />

recommended:<br />

Health Level 7 (HL7): HL7 is a flexible st<strong>and</strong>ard by which various health care systems can<br />

communicate with each other; it is typically used <strong>for</strong> transmission of patient level data.<br />

There are no well-established st<strong>and</strong>ards <strong>for</strong> aggregate data or <strong>in</strong>dicator transmission; however<br />

there are two that are emerg<strong>in</strong>g:<br />

i. Quality Report<strong>in</strong>g Document Architecture (QRDA) 19 is be<strong>in</strong>g developed on the HL7<br />

Cl<strong>in</strong>ical Document Architecture model.<br />

ii. Statistical Data <strong>and</strong> Metadata Exchange (SDMX-HD) 20 is a data exchange <strong>for</strong>mat.<br />

SDMX-HD has been developed by WHO, based on the ISO SDMX report<strong>in</strong>g st<strong>and</strong>ard, to<br />

facilitate exchange of <strong>in</strong>dicator def<strong>in</strong>itions <strong>and</strong> data <strong>in</strong> aggregate data systems.<br />
