Monitor<strong>in</strong>g is an ongo<strong>in</strong>g, cont<strong>in</strong>uous process of collect<strong>in</strong>g data at multiple po<strong>in</strong>ts dur<strong>in</strong>g the<br />

implementation process to ascerta<strong>in</strong> whether set implementation milestones are met or not<br />

whereas evaluation measures how well the EMR implementation objectives have been met 30 .<br />

Evaluation aims at determ<strong>in</strong><strong>in</strong>g how well the ma<strong>in</strong> goals of an EMR system have been met by<br />

answer<strong>in</strong>g the follow<strong>in</strong>g questions:<br />

a) Does the EMR improve patient care?<br />

b) Does the EMR improve record keep<strong>in</strong>g?<br />

c) Does the EMR improve report<strong>in</strong>g?<br />

M&E is a cont<strong>in</strong>uous process <strong>and</strong> should be conducted through the entire life of the EMR <strong>and</strong> set<br />

<strong>in</strong>tervals. M&E assessments seek to ascerta<strong>in</strong> that implementations <strong>and</strong> operations are on track <strong>and</strong><br />

that st<strong>and</strong>ards are adhered to. M&E activities <strong>for</strong> EMR systems aim at generat<strong>in</strong>g lessons learned<br />

<strong>and</strong> best practices that <strong>in</strong><strong>for</strong>m further EMR roll out ef<strong>for</strong>ts.<br />

Different types of monitor<strong>in</strong>g <strong>and</strong> evaluation strategies <strong>and</strong> methodologies may be employed <strong>for</strong><br />

M&E.<br />

1. All EMR implementations shall have a monitor<strong>in</strong>g <strong>and</strong> evaluation plan to guide<br />

implementation <strong>and</strong> evaluate successes <strong>and</strong> failures.<br />

2. The implementation team should identify <strong>and</strong> document key process milestones that<br />

will be used to monitor the EMR <strong>in</strong>stallations.<br />

3. Additionally, the team should def<strong>in</strong>e key questions <strong>and</strong> outcome <strong>in</strong>dicators <strong>for</strong> periodic<br />

evaluation of the EMR systems based on the overall objectives <strong>and</strong> expectations.<br />

Annex H provides a draft Monitor<strong>in</strong>g <strong>and</strong> Evaluation Plan whereas Annex I offers guidance on how<br />

to evaluate the use of an EMR system with<strong>in</strong> the functional areas of an HIV cl<strong>in</strong>ic.<br />
