i. EMR systems<br />

ii. Patient Identifier Cross-reference Consumer actor <strong>for</strong> patient identification which allows<br />

multiple systems us<strong>in</strong>g different patient identifier doma<strong>in</strong>s to determ<strong>in</strong>e patient<br />

identification from a different patient identifier doma<strong>in</strong> us<strong>in</strong>g a cross-referenc<strong>in</strong>g manager.<br />

iii. PIS<br />

In the <strong>in</strong>teraction between EMR system <strong>and</strong> PIS, priority activities are correct patient identification,<br />

medication prescription <strong>and</strong> dispens<strong>in</strong>g. The EMR system, which is the primary actor, serves to<br />

identify patients, place prescription orders <strong>and</strong> receive dispens<strong>in</strong>g <strong>in</strong><strong>for</strong>mation. The EMR system<br />

requires the cooperation of the Patient Identifier Cross-reference consumer actor to provide<br />

support.<br />

There<strong>for</strong>e, the three use cases that can be implemented <strong>in</strong> this EMR-PIS profile are:<br />

i. Dispens<strong>in</strong>g only – the pharmacist receives a paper prescription <strong>and</strong> creates a filler<br />

prescription <strong>in</strong> the PIS <strong>for</strong> dispens<strong>in</strong>g.<br />

ii. Prescription <strong>and</strong> dispens<strong>in</strong>g - Prescription placed by a cl<strong>in</strong>ician. The prescription is<br />

electronically transmitted to the pharmacy.<br />

iii. Prescription, dispens<strong>in</strong>g <strong>and</strong> PIS identification of patients - Order placed by a cl<strong>in</strong>ician,<br />

electronic transmission of prescriptions, the PIS has the ability to query the EMR system <strong>for</strong><br />

patient demographics, with EMR act<strong>in</strong>g as a Master Patient Index (MPI), or directly from a<br />

MPI separate from the EMR system.<br />

The <strong>in</strong><strong>for</strong>mation <strong>in</strong>teraction between PIS <strong>and</strong> EMR can be summarized as follows;<br />

i. Patient Identification - Based on the Patient Identifier Cross-referenc<strong>in</strong>g (PIX) <strong>and</strong> /Patient<br />

Demographic Query (PDQ) profiles<br />

ii. Prescription transmission<br />

iii. Dispens<strong>in</strong>g <strong>in</strong><strong>for</strong>mation transmission<br />

In an ideal environment, the <strong>in</strong>teractions listed above would be implemented <strong>in</strong> the order listed,<br />

with patient identification first, followed by transmission of prescription <strong>in</strong><strong>for</strong>mation, <strong>and</strong> f<strong>in</strong>ally<br />

the transmission of dispens<strong>in</strong>g <strong>in</strong><strong>for</strong>mation. However, <strong>in</strong> a resource-constra<strong>in</strong>ed sett<strong>in</strong>g, greater<br />

cl<strong>in</strong>ical value is accrued by implement<strong>in</strong>g the above <strong>in</strong>teractions <strong>in</strong> reverse order, start<strong>in</strong>g with<br />

transmission of dispens<strong>in</strong>g <strong>in</strong><strong>for</strong>mation, followed by transmission of prescription <strong>in</strong><strong>for</strong>mation <strong>and</strong><br />

patient identification. Description of the identified profiles there<strong>for</strong>e follows this order.<br />
