The duties <strong>of</strong> the lockout committee shall be:<br />

a. Review the overall energy control program;<br />

b. Participate in. or assure the development <strong>of</strong> equipment specific procedures;<br />

c. Assure the selection, development and implementation <strong>of</strong> and where appropriate participate<br />

in training;<br />

d. Review training completeness and effectiveness;<br />

e. Review inspection reports;<br />

f. Assure retraining is accomplished as required.<br />

6. General <strong>Lockout</strong> Procedure.<br />

***COMMENT: The following general procedure is a shortened version <strong>of</strong> an actual corporate procedure.<br />

The principles <strong>of</strong> this procedure apply to the development <strong>of</strong> the specific procedures for particular pieces <strong>of</strong><br />

equipment and tasks. Procedures shall be updated whenever changes to equipment or processes are<br />

made.<br />

a. Employees whose duties require them to installs service or repair machinery, equipment,<br />

machine tools, facility services or any process system shall be provided with sufficient locks<br />

to lockout all energy sources in order to safely perform their job. The guidelines below shall<br />

be followed during the neutralizing <strong>of</strong> energy sources.<br />

b. Before installing, servicing or repairing any <strong>of</strong> the areas mentioned above, where a body part<br />

may possibly be exposed to a point <strong>of</strong> operation, the employee(s) shall completely neutralize<br />

the energy and lockout all energy sources.<br />

c. In general, lockout will be accomplished according to a specific procedure developed and<br />

written for each piece <strong>of</strong> equipment. Procedures will be posted on the machine/equipment/<br />

process as close as possible to the controls.<br />

d. Employees whose duties require them to lockout a machine, systems, facilities or equipment<br />

shall do so after prior notification to operating personnel.<br />

e. Prior to engaging in any repair, service or installation work, the assigned employee(s) shall<br />

neutralize and lockout the energy sources, and check to assure that there is no energy at the<br />

area where the job is to be performed.<br />

f. The neutralizing <strong>of</strong> energy sources may be accomplished by one or more <strong>of</strong> the following:<br />

opening circuits and locking switchboxes, panel disconnects, etc-, closing, bleeding and<br />

padlocking valves; discharging electrical capacitors; mechanically blocking any load or<br />

machine part before working in, on, or under it.<br />

***COMMENT: OSHA does not consider pushbuttons, selector switches, and other control circuit type<br />

devices to be "Energy Isolating Devices”; therefore, locking emergency stop buttons (E-stops),<br />

electrical interlock plugs, or similar control circuit devices does not meet lockout standard<br />

requirements.<br />

g. If a control system cannot be neutralized because it is damaged, it shall be repaired so that<br />

energy neutralization can be accomplished before any further work can be performed, or<br />

alternate energy controls which provide equal protection shall be implemented.<br />
