1. General. An abandoned lockout device may be removed only if this procedure is followed. An<br />

Abandoned <strong>Lockout</strong> Device Removal Permit must be completed by a qualified management<br />

representative and the employee's immediate supervisor (See Appendix A for sample Permit Form<br />

6). These permits will be reviewed periodically by the Local Joint Committee and will be retained for<br />

one year from the date <strong>of</strong> issuance.<br />

2. The qualified management representative for each shift is identified at the end <strong>of</strong> this document.<br />

(These entries shall change whenever the identity <strong>of</strong> the qualified management representative(s)<br />

changes.)<br />

3. When- it is necessary to remove an apparently abandoned lockout device, the following procedure<br />

will be initiated. The qualified management representative and the immediate supervisor shall verify<br />

that the employee whose lockout device(s) has not been removed has left the facility. The<br />

management representative and the immediate supervisor shall:<br />

(a)<br />

(b)<br />

Visually inspect the area, machine, equipment or process to ensure that the employee has<br />

left the facility.<br />

If the employee is not in the facility, the management representative shall:<br />

(i)<br />

(ii)<br />

(iii)<br />

Check time card, or other record, to determine if employee has left the facility.<br />

If time card, or other record, indicates employee has NOT left the facility, the<br />

following steps shall be taken:<br />

(A)<br />

(B)<br />

(C)<br />

Contact fellow workers; use available communication means (pagers, plant<br />

phones, loudspeakers, etc.) to determine whether the employee is still in the<br />

facility.<br />

Physically attempt to locate employee.<br />

Attempt to contact employee at home (by telephone, etc.) until positive<br />

contact can be established.<br />

If the management representative and the immediate supervisor have determined<br />

that the employee has left the facility, they shall:<br />

(A)<br />

(B)<br />

(C)<br />

Attempt to contact employee at home (by telephone, etc.) until positive<br />

contact has been established.<br />

Upon positive contact, notify employee that lockout device(s) is to be<br />

removed and give the reasons for the removal.<br />

Where positive contact has not been made:<br />

** the management representative and immediate supervisor shall notify the<br />

authorized local union representative on that shift that the lockout device(s)<br />

is to be removed, and be given the opportunity to be present at the removal<br />

<strong>of</strong> the lockout device(s). In addition, review with the representative the<br />

steps that have been taken to locate the individual(s).<br />

** the management representative and immediate supervisor shall continue<br />

efforts to make contact and inform the employee <strong>of</strong> the lockout device(s)<br />

removal.<br />

** if contact has not been established before the start <strong>of</strong> the employee's next<br />

scheduled shift, the employee shall receive immediate notification upon<br />

reporting to the facility for the next scheduled shift. The employee shall not<br />

be permitted to undertake any lockout task until such notification has been<br />

given.<br />
