(3) Inspections will not be announced to exposed employees. affected employees. or<br />

supervisors subject to inspection.<br />

a. Inspections will be planned based on a review <strong>of</strong> the specific equipment/task<br />

procedures The plan shall include enough observations to be representative<br />

<strong>of</strong> performance <strong>of</strong> authorized and exposed employees performing each<br />

aspect <strong>of</strong> lockout procedures each year, and frequent or high risk<br />

equipment/task procedure.<br />

b. The performance <strong>of</strong> employees <strong>of</strong> each supervisor performing a<br />

representative number <strong>of</strong> tasks will be observed each quarter. Deficiencies<br />

in performance will trigger retraining for all employees <strong>of</strong> that supervisor. as<br />

well as other necessary corrective action. Reinspection <strong>of</strong> employees <strong>of</strong><br />

that supervisor will occur within one month <strong>of</strong> corrective action. Reinspections<br />

triggered in response to deficiencies shall satisfy the quarterly<br />

inspection requirements.<br />

c. An inspection <strong>of</strong> a specific equipment/task procedure implementation will be<br />

observed promptly on request.<br />

d. The inspection plan will be reviewed with the union bargaining committee<br />

prior to implementation.<br />

e. The progress and results <strong>of</strong> the inspection program will be reviewed at least<br />

quarterly with union representatives, and necessary adjustments made.<br />

(4) Results <strong>of</strong> inspections will be recorded on the inspection report form. (See Sample<br />

Forms No. 4 and No. 5 in Appendix A).<br />

(5) No discipline will result from observations made during energy control inspections<br />

required under the program. Those found in violation should be stopped and<br />

counseled.<br />

(6) Employees will not be required to sign documentation <strong>of</strong> inspections.<br />

(7) A listing <strong>of</strong> corrective actions recommended and completed will be maintained and<br />

made available to the union and to authorized and affected employees.<br />

B. Duties Of Inspectors<br />

The Local Joint Health and Safety Committee shall provide direction to inspectors regarding<br />

the scope, frequency and duration <strong>of</strong> inspections <strong>of</strong> lockout application and/or lockout<br />

removal procedures.<br />

Inspections should be random, and <strong>of</strong> sufficient number and distribution to assess the need<br />

for changes in procedures or for the provision <strong>of</strong> additional training.<br />

Completed forms shall be forwarded to the Local Joint Health and Safety Committee for<br />

review, necessary action, and retention.<br />

12. Refitting <strong>of</strong> equipment to be capable <strong>of</strong> being locked out<br />

(a)<br />

All machinery, equipment and processes must be capable <strong>of</strong> being locked out. Where<br />

machinery, equipment and processes are not equipped with conveniently located<br />

disconnects capable <strong>of</strong> accepting lockout devices, they must be refitted. In the meantime,<br />

special procedures should be developed, posted and communicated to the appropriate<br />

workers.<br />
