Audit Checklist<br />

Review the standard, 29 CFR 1910.147, before beginning. Logout/<strong>Tagout</strong> violations are among<br />

the most common citations made by OSHA.<br />

As a quick gauge <strong>of</strong> where your program stands in terms <strong>of</strong> compliance and employee safety,<br />

evaluate the following items. Even better, have your employees evaluate the program. They<br />

may identify weaknesses you have never thought <strong>of</strong>.<br />

Yes No Is a copy <strong>of</strong> 29 CFR 1910.147 open and available to employees and<br />

supervisors for reference as needed? This can be very helpful as to<br />

requirements with unusual processes or operations.<br />

Yes No Has comprehensive training been conducted with all employees who will<br />

utilize LOTO?<br />

Yes No Were supervisors and plant management also trained to a predetermined<br />

level <strong>of</strong> competence?<br />

Yes No Were management/supervision made aware <strong>of</strong> specific liability and<br />

potential damages <strong>of</strong> noncompliance <strong>of</strong> LOTO?<br />

Yes No Is training conducted as needed to update employees/management on<br />

new procedures or operations?<br />

Yes No Has the LOTO program been evaluated by an outside source to ensure<br />

compliance with your company policy? Sometimes it is difficult to judge<br />

your own compliance.<br />
