Hot water!<br />

Chaltu is immuniz<strong>in</strong>g children when her supervisor arrives unexpectedly. Hav<strong>in</strong>g<br />

prepared to give her next <strong>in</strong>jection, the supervisor picks up some DPT from a cup of<br />

water. The supervisor feels the water and f<strong>in</strong>ds that it is hot.<br />

The supervisor asks, "Why is this vacc<strong>in</strong>e <strong>in</strong> hot water?"<br />

"The vacc<strong>in</strong>e was frozen solid this morn<strong>in</strong>g when I took it out of the refrigerator,"<br />

says Chaltu. "I had to put it <strong>in</strong> hot water to melt it so that I could get it <strong>in</strong>to the<br />

syr<strong>in</strong>ge."<br />

Discussion<br />

1. What has Chaltu done that is wrong?<br />

Chaltu should not immunize children with DPT that has been frozen. It has<br />

lost its potency and cannot make them immune. Melt<strong>in</strong>g frozen vacc<strong>in</strong>e <strong>in</strong> hot<br />

water will not make it potent aga<strong>in</strong>.<br />

2. What should be done with the DPT vacc<strong>in</strong>e?<br />

The frozen vials should be discarded. Chaltu should check the other DPT<br />

vials <strong>in</strong> the refrigerator to see whether any of them are frozen. He should<br />

make sure that none of them are be<strong>in</strong>g stored on the top shelf. He should<br />

also check the temperature of his refrigerator. It is probably too cold, <strong>in</strong><br />

which case he should adjust the thermostat.<br />

3. What should Chaltu do about the children to whom she gave the DPT?<br />

Chaltu should f<strong>in</strong>d the children he has immunized with frozen DPT and<br />

immunize aga<strong>in</strong>.<br />
