3. Specimen Collection Label.<br />

• Use this form to record data when sea lion or<br />

Northern fur seal canine teeth (snouts) have been<br />

collected.<br />

The Marine Mammal Sighting Form should be completed<br />

whether or not the marine mammal is alive or dead and<br />

whether the animal has been brought on board the fishing<br />

vessel or not. The Biospecimen Form only needs to be<br />

completed for dead marine mammals onboard a fishing<br />

vessel.<br />

Biospecimen Form Instructions<br />

Complete the Biospecimen Form any time length, sex or<br />

weight information is collected from a dead marine mammal.<br />

Also complete this form if canine teeth or tissue samples have<br />

been collected. An example of the form is included as Figure<br />

7-4.<br />

• Haul Number – Record the number of the haul<br />

that the marine mammal came from.<br />

• Date – Record the date as MM/DD/YY.<br />

• Trip Number – Record the trip number generated<br />

by the database system.<br />

• <strong>Coast</strong> Guard Number – Record the USCG vessel<br />

number on limited entry trawlers and fixed gear<br />

vessels (if they have one). Do not record anything<br />

in this field if you are on an open access vessel or a<br />

limited entry fixed gear vessel that does not have a<br />

USCG number.<br />

• Catch # - Record the number that corresponds to<br />

the catch category on the Catch Form.<br />
