1. Personal statement (no longer than 2-3 pages double-spaced) discussing the
reason for your current semester medical withdrawal. Questions to answer in a
personal statement.
• What occurred?
• How did it affect your ability to continue classes for the current
semester?
• What medical assistance did you seek or receive?
• What university staff or faculty did you consult with during this time?
2. Current medical documentation signed and dated by your physician,
psychologist, or counselor stating the following (letters from parents or legal
guardians will not be accepted):
• Medical diagnosis
• Dates of Treatment
• Nature, duration and diagnosis of medical problem
• How the medical diagnosis interfered with the student's ability to
perform academically
• Recommendation of medical providers whether student should withdraw
from the current semester
• Supporting documents for a traumatic event (e.g. death of family member, acts of
violence, etc.) must include:

2. Copy of death certificate or obituary for your immediate family member
Medical Withdrawal requests will not be considered unless all documentation has been
received by the Vice President of Academic Affairs. The Medical Withdrawal process is not
complete until the student obtains all required signatures, submits all required documentation
and receives a judgment from the VP of Academic Affairs. All Medical Withdrawals must be
completed in person, except in extreme cases of illness or distance.
Dependent upon the date of Medical Withdrawal, tuition charges may still be applicable.

MAXIMUM ACADEMIC LOAD
A full-time course load in a single semester is regarded as twelve (12) or more semester hours.
Students who have earned a 2.50 cumulative GPA or less are strongly advised to limit their
academic load to 12 hours. The maximum full-time load that a student in good standing may
take in a single semester, without special permission, is eighteen (18) semester hours. Before
