18. Individual Service Plans (ISP).
Staff and parents/guardians participate in developing ISP for their child with the Special
Needs Resourcing agency (CCR).
The ISP includes short term objectives derived from the long term goals, both stated in
the ISP.
The ISP must include strategies for the implementation and must be updated on a
regular basis (monthly, every three months, every six months).
The ISP strategies and adaptations are reviewed on an ongoing basis and immediate
changes are made if the plan is not working.
The ISP is completed and incorporated into the overall program plan.
Staff have developed a number of innovative ways to encourage full participation for all
children.
The program plans accommodate all children and ensure that all children have the
opportunity to participate in all activities and play areas.
Program reviews take place on a regular basis to ensure the program is meeting the
needs of individual children.
Appropriate consent forms are signed and in the child’s file.
All ISP’s must be available for all staff to review.
The ISP’s are not to be posted in a way that the child is identified, as well as any
specifics regarding the child or the child’s specific programming.
Staff follow through with activities and interactions which will assist children to reach their
identified goals.
