strEngtHEning tHE DiagnostiC sErviCEs aCross tHE statE<br />
ProblEm statEmEnt<br />
The percentage of pregnant women receiving free<br />
diagnostic services under JSSK in the year 2013-<br />
14 has increased to 62% as compared to 20% in the year<br />
2011-12 as per the State JSSK report. H&FW Department,<br />
government of Odisha has reiterated its commitment<br />
to provide quality, accessible, affordable diagnostic<br />
services to its entire populati<strong>on</strong>. For meeting these<br />
objectives ‘Strengthening Diagnostic Services’ is <strong>on</strong>e of<br />
the key interventi<strong>on</strong>s. Since the JSSK Scheme is limited to<br />
pregnant mothers and infants <strong>on</strong>ly, a large secti<strong>on</strong> of other<br />
beneficiaries has been left-out. Therefore, c<strong>on</strong>sidering the<br />
disease burden in the State and demand of the community,<br />
the diagnostic services at the facilities are proposed to be<br />
strengthened under this initiative.<br />
ProgrammE DEsCriPti<strong>on</strong><br />
Even though various models are used for strengthening<br />
the Lab services, the State of Odisha has focused <strong>on</strong><br />
the System Strengthening model for improvement<br />
of diagnostic services across the State. The<br />
advantages of System strengthening initiative are<br />
Sustainable, Cost-effectiveness and Wide coverage in<br />
rural areas.<br />
Modality of Implementati<strong>on</strong><br />
For achieving improvement in the availability of<br />
diagnostic services, it is proposed to strengthen the<br />
laboratories at DH and CHC levels by transforming them<br />
into “integrated labs” where all the diagnostic-related<br />
services are intended to be provided as <strong>on</strong>e-stop. Services<br />
through integrated labs will be provided till CHC level<br />
whereas below the CHC level more focus will be given<br />
<strong>on</strong> kit-based tests. The activities for establishment of<br />
integrated labs are planned to be d<strong>on</strong>e in three different<br />
levels which are listed below:<br />
Level 1: Seeking approvals for all the necessary plans<br />
prepared for integrated labs.<br />
Level 2: Providing the infrastructure (manpower,<br />
equipment) for the functi<strong>on</strong>ality of the programme<br />
as per the design approved through Pooling<br />
of Resources and Automati<strong>on</strong> of equipment<br />
instruments.<br />
Level 3: Full scale Implementati<strong>on</strong> across the State.<br />
ProgrammE outComEs<br />
The interventi<strong>on</strong> is under various stages of implementati<strong>on</strong><br />
and its full impact is yet to be assessed.<br />
sCalabilitY<br />
• The State intents to prepare Standard Operating<br />
Procedures (SOP) with focus <strong>on</strong> quality c<strong>on</strong>trol<br />
and biomedical waste management, focus <strong>on</strong><br />
primary healthcare-based diagnostic services<br />
through the use of rapid testing kits at PHCs and SCs.<br />
The State has proposed to provide round-the-clock<br />
services with also appropriate iT-enabled reporting<br />
systems.<br />
C<strong>on</strong>Clusi<strong>on</strong>s/lEss<strong>on</strong>s lEarnt<br />
H&FW department of the State intends investing<br />
more resources to provide impetus to ensure quality<br />
laboratory services despite issues and challenges related<br />
to deployment and availability of skilled manpower,<br />
interrupted power supply, and supply of equipment.<br />
In the coming years, it is expected that by putting<br />
adequate resources, str<strong>on</strong>ger quality and regulatory<br />
mechanisms, usage of IT and putting the right structures<br />
in place, diagnostic services will be implemented<br />
effectively.<br />
