Overall instituti<strong>on</strong>al deliveries in the state reached to 97.3% in 2014-15. The maternal<br />
deaths in the state also showed a decline from 25 in 2011-12 to 12 in 2014-15<br />

ProbleM stateMent<br />

A and an increasing problem of n<strong>on</strong> communicable<br />
diseases in the State.<br />
PrograMMe DescriPti<strong>on</strong><br />
A cause of death analysis in 2012 by the State showed that<br />
over half of deaths were <strong>on</strong> account of complicati<strong>on</strong>s of<br />
n<strong>on</strong> communicable diseases (NCD). Given that life styles<br />
are a key c<strong>on</strong>tributor in the etiology of several NCDs, the<br />
State undertook training of members of the Village <strong>Health</strong>,<br />
Sanitati<strong>on</strong> and Nutriti<strong>on</strong> Committees (VHSNC) in the year<br />
2014 in order to improve the community’s health seeking<br />
behavior through community participati<strong>on</strong>. VHSNC<br />
members including the PRI representative, ASHAs,<br />
AWWs, NGO members, representatives from Self Help<br />
Group, teachers and religious leaders were trained at the<br />
PHC level. This effort builds <strong>on</strong> an existing initiative in the<br />
State, namely the CATCH scheme.<br />
The State already has in place a CATCH scheme (Chief<br />
Minister’s Comprehensive Annual and Total Checkup<br />
for <strong>Health</strong>y Sikkim). The scheme involves community<br />
mobilizati<strong>on</strong> of the populati<strong>on</strong> sub group by the ASHA and<br />
the administrati<strong>on</strong> of a screening tool by the ANM. This<br />
is followed by an examinati<strong>on</strong> c<strong>on</strong>ducted by the Medical<br />
officer, who then prescribes the requisite medicati<strong>on</strong> or<br />
enables referral. The State assures treatment for all patients<br />
requiring services at no cost, either within or outside the<br />
State. The CATCH survey is c<strong>on</strong>ducted annually. The survey<br />
includes questi<strong>on</strong>s related to Family History, lifestyle<br />
related (diet, tobacco and alcohol c<strong>on</strong>sumpti<strong>on</strong>), insurance<br />
coverage, Reproductive history, and includes all family<br />
members including infants. Diagnostic screening tests<br />
include: Hb, Random Blood glucose, Serum Cholesterol/<br />
Triglycerides, and Creatinine. Electr<strong>on</strong>ic <strong>Health</strong> Records<br />
(HER) cards have also been issued.<br />
During the training the roles and resp<strong>on</strong>sibilities of VHSNC<br />
using the ward wise/sub center wise presentati<strong>on</strong> <strong>on</strong><br />
findings of the CATCH scheme and other health indicators<br />
were discussed. The trainees were presented with the data<br />
<strong>on</strong> home delivery, major diseases according to CATCH and<br />
causes of death. This enabled the development of acti<strong>on</strong><br />
plans with the ASHA and PRI member taking ownership<br />
for the acti<strong>on</strong>s that were decided by the group.<br />
PrograMMe outcoMes<br />
Increased community participati<strong>on</strong> led by ASHAs resulted<br />
in increase in instituti<strong>on</strong>al deliveries and improved<br />
reporting and detecti<strong>on</strong> of NCDs. There is some evidence<br />
to show that sec<strong>on</strong>dary preventi<strong>on</strong> measures are working<br />
as well.<br />
financial investMent<br />
No additi<strong>on</strong>al costs were incurred as members were<br />
trained during VHNSC training.<br />
scalability<br />
The State has utilized the existing resources of VHSNC<br />
members by training and planning using existing data.<br />
Therefore training of VHSNC members in addressing an<br />
identified local problem using existing resources can be<br />
scaled up across other States.<br />
c<strong>on</strong>clusi<strong>on</strong>s/less<strong>on</strong>s learnt<br />
The positive results of the interventi<strong>on</strong> have dem<strong>on</strong>strated<br />
that it is feasible to address local challenges faced by the<br />
community by strengthening community participati<strong>on</strong>.<br />
Joint training facilitated better coordinati<strong>on</strong> between PRI<br />
members, <strong>Health</strong> workers, ASHA, NGO representatives<br />
and other community members. While currently available<br />
evidence cannot definitively State that this is directly<br />
attributable, change in health behaviors fostered by such<br />
community acti<strong>on</strong> and planning, could lead to positive<br />
impact.<br />
