ProbleM stateMent<br />
Percepti<strong>on</strong> of poor quality of health care dissuade<br />
patients from using the available services. Most of<br />
the comm<strong>on</strong> causes for maternal mortality are Postpartum<br />
haemorrhage and eclampsia and three major causes<br />
c<strong>on</strong>tribute to about 75 per cent of all deaths in the newborn<br />
period: pre-maturity, birth asphyxia and infecti<strong>on</strong>s.<br />
PrograMMe DescriPti<strong>on</strong><br />
a. Start Date: September 2009 for NABH, kASH,<br />
Guidelines for MMR and IMR in 2012, and NQAS<br />
in 2013.<br />
b. Kerala Government has initiated the following Quality<br />
Assurance programme for the Government Hospitals:<br />
i. Nati<strong>on</strong>al Accreditati<strong>on</strong> Board for Hospitals and<br />
<strong>Health</strong> care organizati<strong>on</strong>.<br />
ii. State Level Accreditati<strong>on</strong> Programme- kerala<br />
Accreditati<strong>on</strong> Standards for hospitals.<br />
iii. Nati<strong>on</strong>al Standards <strong>on</strong> Quality Assurance<br />
published by Ministry of <strong>Health</strong> and Family<br />
Welfare, government of India.<br />
iv. Measures to reduce MMR in kerala.<br />
v. Measures to reduce IMR in kerala.<br />
c. Six Government hospital and <strong>on</strong>e government blood<br />
bank in kerala was accredited by Nati<strong>on</strong>al Accreditati<strong>on</strong><br />
Board for Hospitals and <strong>Health</strong> care organizati<strong>on</strong>.<br />
d. The Kerala Accreditati<strong>on</strong> Standards for Hospitals<br />
(KASH) are prepared for 4 different levels of hospital,<br />
which are Primary <strong>Health</strong> Centre (PHC), Community<br />
<strong>Health</strong> Centre (CHC), Taluk level Hospitals (THQH)<br />
and District level Hospitals including specialty and<br />
General Hospitals. These standards are framed<br />
after referring the accreditati<strong>on</strong> programmes in<br />
different countries and other existing accreditati<strong>on</strong><br />
and certificati<strong>on</strong> programmes. So far 17 health care<br />
organizati<strong>on</strong>s are accredited under kASH.<br />
e. The <strong>Health</strong> Services Department also started to<br />
implement the Nati<strong>on</strong>al Standards <strong>on</strong> Quality<br />
Assurance developed by the Ministry of <strong>Health</strong> and<br />
Family Welfare, Government of India. The standards<br />
are being implemented in selected hospitals in the<br />
<strong>Health</strong> Services Department.<br />
f. A quality standard document has been prepared for<br />
reducing the maternal death during the delivery<br />
services. Quality standards that are derived from<br />
evidence-based clinical guidelines and that are agreed<br />
by relevant stakeholders provide powerful levers to<br />
drive and measure quality improvement in health care<br />
instituti<strong>on</strong>s. It focuses <strong>on</strong> improving the care mothers<br />
receive in hospitals (public and private) to help reduce<br />
maternal mortality, <strong>on</strong>e of the main health priorities<br />
in Kerala. Outcome of the implementati<strong>on</strong> of Quality<br />
standards in selected hospitals has resulted in a<br />
decrease in the PPH and decreased referral to higher<br />
centres. The patients were stabilised before the referral<br />
and decrease in the use of blood products.<br />
g. The Quality Standards was prepared for the major<br />
causes of IMR. It is expected that implementati<strong>on</strong> of<br />
Quality Standards in the antenatal and paediatrics<br />
practice may lead to further reducti<strong>on</strong> in IMR, which<br />
has stagnated in the State.<br />

Accreditati<strong>on</strong> to NABH Standards may be expensive, which<br />
other States may find difficult to emulate. Certificati<strong>on</strong> to<br />
KASH and Nati<strong>on</strong>al Quality Assurance Standards would<br />
be scalable, which is being followed in other States.<br />
c<strong>on</strong>clusi<strong>on</strong>s/less<strong>on</strong>s learnt<br />
State has introduced many quality assurance programmes<br />
for the improvement of the quality of services been<br />
provided in the public health facilities. However its full<br />
impact <strong>on</strong> outcome is yet to be evaluated. The Ministry<br />
of <strong>Health</strong> & Family Welfare has already evaluated<br />
NABH Accreditati<strong>on</strong> process through an independent<br />
organisati<strong>on</strong> in the year 2011-12. The NABH process was<br />
found expensive and time-c<strong>on</strong>suming.<br />
