roll-out oF nati<strong>on</strong>al qualitY assuranCE ProgrammE<br />
at DistriCt HosPitals<br />
ProblEm statEmEnt<br />
Quality of Services at Public <strong>Health</strong> Facilities has been<br />
cause of c<strong>on</strong>cern. The envisaged system for Quality<br />
Assurance was expected to be internalised, flexible, less<br />
resource intensive and addressing the needs of Public<br />
<strong>Health</strong> System in the State.<br />
ProgrammE DEsCriPti<strong>on</strong><br />
After launch of the ‘Operati<strong>on</strong>al Guidelines for Quality<br />
assurance in Public <strong>Health</strong> Facilities’ by the MoHFW gOi<br />
in Nov 2013, the programme has started in the State with<br />
the objective completing baseline assessment of all district<br />
hospitals, without waiting for inducti<strong>on</strong> of the additi<strong>on</strong>al<br />
HR into the Quality Assurance Team. So that acti<strong>on</strong>s for<br />
the gap-closure starts in full earnest. For completing the<br />
assessment quickly, a 4-5 members assessment team,<br />
c<strong>on</strong>sisting of Development Partners, Hospital Managers,<br />
DH functi<strong>on</strong>aries, RPMUs, DPMUs (under NRHM) was<br />
formed in Dec 2013 with Deputy Superintendent having<br />
a pivotal role in the process. Base-line assessment of all<br />
district hospitals was completed in two phases. First phase<br />
was c<strong>on</strong>ducted from 1st June 2014 to 30th June 2014. Total<br />
17 Sadar Hospitals + MCH Unit of PMCH and SKMCH<br />
covered in this phase. Sec<strong>on</strong>d phase was c<strong>on</strong>ducted for<br />
19 Sadar Hospitals from 1st October 2014 to 20th October<br />
2014. Findings of baseline assessment has been categorised<br />
and resource allocati<strong>on</strong> has been made. For strengthening<br />
labour rooms of 149 FRUs resources has also been allocated<br />
under Quality Assurance Programme.<br />
in order to c<strong>on</strong>solidate the gains made, 48 hospitals are<br />
being taken-up for the Nati<strong>on</strong>al level QA certificati<strong>on</strong>.<br />
ProgrammE outComEs<br />
• improved external appearance of the health facility.<br />
• Cleaner envir<strong>on</strong>ment around and inside of the health<br />
facility.<br />
• Display of signages and clear informati<strong>on</strong> about<br />
services, incentives and availability of free drugs.<br />
• As per SOPs, services & service delivery practices are<br />
carried out.<br />
• Functi<strong>on</strong>ing equipment.<br />
• Reduced waiting times for registrati<strong>on</strong> and<br />
c<strong>on</strong>sultati<strong>on</strong>s.<br />
• Increased uptake for both major and minor surgery<br />
increased OPD, deliveries in hospitals, better inpatient<br />
care.<br />
• improved and rati<strong>on</strong>alised record keeping.<br />
Evaluati<strong>on</strong><br />
External evaluati<strong>on</strong> of the programme initiative has not<br />
yet been taken.<br />
sCalabilitY<br />
There are 17 district hospitals which have been proposed<br />
for the Nati<strong>on</strong>al certificati<strong>on</strong>. Baseline Assessment Process<br />
has already started in 30 FRU’s and being prepared for the<br />
Nati<strong>on</strong>al certificati<strong>on</strong>.<br />
C<strong>on</strong>Clusi<strong>on</strong>s/lEss<strong>on</strong>s lEarnt<br />
Implementati<strong>on</strong> of Quality Assurance at Public <strong>Health</strong><br />
Facilities needs harnessing available resources, creati<strong>on</strong><br />
of enabling envir<strong>on</strong>ment, and supportive supervisi<strong>on</strong>.<br />
Patient satisfacti<strong>on</strong> appears to have improved besides<br />
improvement in Staff morale.<br />
