Otitis media and externa KELLY YOUNG n Agents n NSAIDs n Acetaminophen n Topical local anesthetics n Topical naturopathics n Topical antihistamines n Topical corticosteroids n Opioids n Evidence Given the frequency of otitis media (OM) and otitis externa (OE), there is surprisingly little evidence on treating pain associated with these conditions. For both conditions, there can be utility in mechanical interventions (myringotomy for OM, ear canal cleaning and drying for OE). An additional part of otalgia control is patient instruction. Patients with OM should avoid pressure changes (e.g. flying), and those with OE need to avoid further ear canal irritation (e.g. swimming). As for pharmacotherapy, guidelines emphasize the importance of pain management, but recommendations are based on expert panel consensus rather than methodologically rigorous comparative trials. 1,2 The NSAIDs are typically recommended as first-line treatment, although evidence is sparse. One RCT in young children found that ibuprofen and acetaminophen (paracetamol) similarly outperformed placebo in terms of OM pain relief. 3 While the analgesic benefit of the NSAID (like that of acetaminophen) seems likely, the effect magnitude is probably small since two smaller trials failed to identify any benefit (compared with placebo) for naproxen use in painful OM. 4,5 Topically applied local anesthetics, generally benzocaine, are widely used for otalgia. The evidence is limited, but available data from one study indicate 335
