ates in children aged 6 to 10 years. 12 In another study, characterized by low overall rates of analgesia, Selbst et al. found that 12% of children less than 24 months received analgesics, compared with 38% of children aged 2 to 18 years. 3 SEX Epidemiologic overview of ED pain treatment 37 Two retrospective studies suggest that women receive significantly more analgesics than men. 13,14 However, the preponderance of evidence argues against gender-related pain treatment. The differing conclusions in available studies may result from methodology. In a rigorous prospective study, univariate analysis showed that women were more likely to receive pain medication than men but this finding was nullified with use of multivariate correction for factors such as patient and physician perception of pain severity. 15 Several other studies also fail to find an association between patient gender and pain treatment. 7,14,16 n Discussion: lessons for the acute care provider It is heartening that, over the past few decades, the approximate proportion of adult patients in pain receiving analgesics has increased from 40% to 70%. While it is true that over a fourth of patients in pain still do not receive any pain medication, it is not clear that “adequate pain treatment” means that all patients in pain should receive analgesics. There are patients who do not want pain medication, and instances in which the use of analgesics is illadvised. However, nearly half of patients who do not receive analgesia indicate they do want treatment, and the modest 18% increase in analgesia use over 1997–2001 (based upon NHAMCS data) suggests that we are only beginning to move in the right direction. There are grounds for optimism in the data, which suggests that racial/ ethnic disparities in analgesia provision are diminishing with the passage of time. The literature should be interpreted with the caveat that most of the studies deal with extremity fractures. The long-bone fracture model is attractive
