ended. <str<strong>on</strong>g>The</str<strong>on</strong>g> disrupti<strong>on</strong> and increased mortality that<br />

persist at the end <str<strong>on</strong>g>of</str<strong>on</strong>g> a violent c<strong>on</strong>flict need to be<br />

taken seriously into account in the planning <str<strong>on</strong>g>of</str<strong>on</strong>g> l<strong>on</strong>gterm<br />

rec<strong>on</strong>structi<strong>on</strong> and development programmes.<br />

GLOBAL BURDEN <str<strong>on</strong>g>of</str<strong>on</strong>g> ARMED VIOLENCE<br />

Without detailed data <strong>on</strong> mortality for all the<br />

c<strong>on</strong>temporary c<strong>on</strong>flicts discussed in the preceding<br />

chapter (DIRECT CONFLICT DEATH), it is not<br />

possible to give a precise estimate <str<strong>on</strong>g>of</str<strong>on</strong>g> the indirect<br />

burden <str<strong>on</strong>g>of</str<strong>on</strong>g> armed violence. But an order <str<strong>on</strong>g>of</str<strong>on</strong>g> magnitude<br />

can be <str<strong>on</strong>g>of</str<strong>on</strong>g>fered for the purposes <str<strong>on</strong>g>of</str<strong>on</strong>g> comparis<strong>on</strong><br />

with other aspects <str<strong>on</strong>g>of</str<strong>on</strong>g> the global burden <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

armed violence, based <strong>on</strong> the following data and<br />

assumpti<strong>on</strong>s:<br />

<str<strong>on</strong>g>The</str<strong>on</strong>g> direct death burden in c<strong>on</strong>flicts for 2004–<br />

07 from incident reporting was 208,300, or<br />

about 52,000 per year. 12 <str<strong>on</strong>g>The</str<strong>on</strong>g>se reported<br />

deaths clearly undercount the actual total <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

direct c<strong>on</strong>flict deaths, although the degree <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

undercounting varies by c<strong>on</strong>flict.<br />

A previous study <str<strong>on</strong>g>of</str<strong>on</strong>g> undercounting in specific<br />

c<strong>on</strong>flicts dem<strong>on</strong>strated that it could be between<br />

two and four times the level captured in incident<br />

reports (Obermeyer et al., 2008; Small<br />

Arms Survey, 2005, p. 230). In the DRC al<strong>on</strong>e,<br />

an estimated average <str<strong>on</strong>g>of</str<strong>on</strong>g> 51,000 people have<br />

died violently per year since 1998, although<br />

the annual totals have been lower since 2002.<br />

A c<strong>on</strong>servative ratio <str<strong>on</strong>g>of</str<strong>on</strong>g> 4:1 indirect to direct<br />

deaths would mean that the burden <str<strong>on</strong>g>of</str<strong>on</strong>g> indirect<br />

deaths for an average year between 2004 and<br />

2007 would be at least 200,000 and probably<br />

higher. 13<br />

<str<strong>on</strong>g>The</str<strong>on</strong>g> total number <str<strong>on</strong>g>of</str<strong>on</strong>g> indirect deaths would vary<br />

c<strong>on</strong>siderably from year to year, depending <strong>on</strong> the<br />

number and intensity <str<strong>on</strong>g>of</str<strong>on</strong>g> c<strong>on</strong>flicts, the nature <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

the fighting, the provisi<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> humanitarian assistance,<br />

and the c<strong>on</strong>diti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the affected populati<strong>on</strong>.<br />

In order to avoid the impressi<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> excessive pre-<br />

cisi<strong>on</strong> in what is simply an order <str<strong>on</strong>g>of</str<strong>on</strong>g> magnitude,<br />

this report c<strong>on</strong>cludes that <strong>on</strong> average, at least<br />

200,000 pers<strong>on</strong>s have died each year as an indirect<br />

result <str<strong>on</strong>g>of</str<strong>on</strong>g> c<strong>on</strong>flict since 2004.<br />

<str<strong>on</strong>g>The</str<strong>on</strong>g> pages following the end <str<strong>on</strong>g>of</str<strong>on</strong>g> this chapter provide<br />

detailed discussi<strong>on</strong>s <str<strong>on</strong>g>of</str<strong>on</strong>g> three l<strong>on</strong>g wars—in<br />

Iraq, Sierra Le<strong>on</strong>e, and South Sudan—to provide<br />

c<strong>on</strong>crete illustrati<strong>on</strong>s <str<strong>on</strong>g>of</str<strong>on</strong>g> how field-based surveys<br />

can provide a more adequate picture <str<strong>on</strong>g>of</str<strong>on</strong>g> the burden<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> violence in armed c<strong>on</strong>flicts.<br />

C<strong>on</strong>clusi<strong>on</strong><br />

Quantifying excess mortality and indirect deaths<br />

is a difficult task. But the expert c<strong>on</strong>sensus is<br />

that in almost all c<strong>on</strong>temporary armed c<strong>on</strong>flicts,<br />

indirect deaths are <str<strong>on</strong>g>of</str<strong>on</strong>g>ten more numerous than<br />

mortality arising from violence. N<strong>on</strong>-violent deaths<br />

that can be directly linked to c<strong>on</strong>flict should count<br />

as part <str<strong>on</strong>g>of</str<strong>on</strong>g> the burden <str<strong>on</strong>g>of</str<strong>on</strong>g> armed violence, since<br />

from a human perspective it matters little if a<br />

parent or child dies from a bullet or from dysentery<br />

so<strong>on</strong> after an armed clash.<br />

Several scientifically rigorous methods have been<br />

developed and improved in recent years, by epidemiologists,<br />

demographers, and statisticians, to<br />

provide reliable estimates. <str<strong>on</strong>g>The</str<strong>on</strong>g>se methods c<strong>on</strong>tinue<br />

to be refined and standardized, as evidenced by the<br />

SMART (Standarized M<strong>on</strong>itoring and Assessment<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> Relief and Transiti<strong>on</strong>s) initiative and the general<br />

increase in the quality <str<strong>on</strong>g>of</str<strong>on</strong>g> data collecti<strong>on</strong> and<br />

analysis in humanitarian research.<br />

C<strong>on</strong>tinued innovati<strong>on</strong> in measuring indirect mortality<br />

in c<strong>on</strong>flicts will be crucial to understand<br />

the true human impact <str<strong>on</strong>g>of</str<strong>on</strong>g> mortality in c<strong>on</strong>flicts,<br />

to help set priority public health goals for the<br />

preventi<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> disease and malnutriti<strong>on</strong>, and to<br />

provide the evidence base to hold perpetrators<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> violent acts against innocent populati<strong>on</strong>s<br />

legally accountable.
