Oregon HIV/AIDS Housing and Services Systems Integration Plan—Appendices A-47 GOALS & DESCRIPTIONS Short- and long-term housing combined with services designed to assist people whose health makes independent living impossible Best where there is a large concentration of PLWHA who require higher-end care ASSISTANCE TYPES & HOPWA–ELIGIBLE USES Specialized Care Facilities - Can fund acquisition, rehab, etc. - Can fund facility operations ADVANTAGES DISADVANTAGES - Can provide a high level of care for people whose medical or behavioral health does not allow them to live independently, as an alternative to hospitalization - Very expensive, due to skilled staffing needs - Consumers living longer prefer not to live in specialized care facilities - Only possible in areas with a large concentration of PLWHA
A-48 Appendices—Oregon HIV/AIDS Housing and Services Systems Integration Plan Services to Support Housing Stability One-time assistance for move-in or occupancy or ongoing services to support housing stability GOALS & DESCRIPTIONS Supportive services offered as a complement to housing, such as case management, health care or treatment, food and transportation assistance, etc. Should be an integral part of every housing solution. Requires good referral arrangements with other service providers Assists households in overcoming the one-time challenges of establishing a new residence Essential where homeless people are transitioning into permanent housing ASSISTANCE TYPES & HOPWA–ELIGIBLE USES Support Services* - Services including housing advocacy, home care, counseling, nutrition and meal services, crisis intervention, legal assistance, transportation, day health programs, mental health services, personal assistance, and substance use treatment - Can be offered with or without housing Move-In/Occupancy Needs Assistance* - First month’s rent, deposits and tenant screening fees up to the value of 2 mos. rent under HOPWA supportive services category, permanent housing placement activity ADVANTAGES DISADVANTAGES - Can help tenants remain in their home - Existing providers and volunteer teams can contribute - Less complex than facility-based assistance; few start-up costs required - Relatively inexpensive - Easy to administer - Volunteer teams and local businesses can contribute - Some consumers will also need financial assistance to keep housing. - Providing services in widely scattered locations can be expensive - Securing ongoing funding can be difficult - Many need more assistance - Deposits may be kept by landlords or departing tenants
