Continua <strong>of</strong> Care (CoC)<br />
In order to encourage the integration and coordination <strong>of</strong> community homeless assistance, HUD’s<br />
three primary homeless assistance programs – the Supportive <strong>Housing</strong> Program (SHP), Shelter Plus<br />
Care (S+C), and the Section 8 Moderate Rehabilitation Single Room Occupancy Program (SRO<br />
Mod. Rehab.) – are combined under the Continuum <strong>of</strong> Care (CoC) planning and allocation process.<br />
The establishment <strong>of</strong> a CoC system involves a community-wide or region-wide process involving<br />
government agencies, nonpr<strong>of</strong>it housing and service providers, private foundations, homeless or<br />
formerly homeless individuals, and other stakeholders.<br />
Each CoC is allocated an initial pro rata need funding amount based on the relative homeless<br />
assistance need assigned to a community using U.S. Census Bureau data. Communities can receive<br />
funding above this level through successful applications for “bonus” projects that must provide<br />
permanent housing for chronically homeless individuals.<br />
Figure 2 on the following page shows the map <strong>of</strong> the CoC in <strong>Oregon</strong>, followed by a discussion <strong>of</strong><br />
each CoC in the balance <strong>of</strong> state.
