THE VOLUME CATEGORY OF THE HOSPITAL<br />

Table 6.18 shows the differ<strong>en</strong>ces in case mix by <strong>volume</strong> of c<strong>en</strong>tres. The cut off of<br />

<strong>volume</strong> (219 and 401) divi<strong>de</strong> the 29 c<strong>en</strong>tres in three groups of equal size. These<br />

thresholds correspond more or less to those used by the Ag<strong>en</strong>cy for Healthcare<br />

Research and Quality (upper threshold of 200 CABG per year) and by the Leapfrog<br />

Group (450 CABG per year). 29, 126 More information on these international <strong>volume</strong><br />

Age and g<strong>en</strong><strong>de</strong>r are not associated with <strong>volume</strong>. There are, however, a higher<br />

pr<strong>op</strong>ortion of pati<strong>en</strong>ts with a high Charlson score in low and medium <strong>volume</strong> c<strong>en</strong>tres.<br />

This is confirmed by a higher pr<strong>op</strong>ortion of pati<strong>en</strong>ts with diabetes and r<strong>en</strong>al disease in<br />

these c<strong>en</strong>tres in comparison to high <strong>volume</strong> c<strong>en</strong>tres.<br />

High <strong>volume</strong> c<strong>en</strong>tres perform more combined <strong>op</strong>erations than low <strong>volume</strong> c<strong>en</strong>tres<br />

(18% versus 12%).<br />

L<strong>en</strong>gth of stay (LOS) was also longer in low <strong>volume</strong> c<strong>en</strong>tres (17.5 days) than in high<br />

<strong>volume</strong> c<strong>en</strong>tres (15.1 days).<br />

In-hospital mortality was inversely associated with the <strong>volume</strong> of c<strong>en</strong>tres: 6.7% in low<br />

<strong>volume</strong> c<strong>en</strong>tres, 5.7% in medium <strong>volume</strong> c<strong>en</strong>tres and 3.6% in high <strong>volume</strong> c<strong>en</strong>tres.
