5.4 COLON CANCER SURGERY<br />
5.4.1 G<strong>en</strong>eral <strong>de</strong>scription of procedure<br />
Colectomy, which is a surgical procedure to remove the section of colon containing<br />
cancer and nearby lymph no<strong>de</strong>s, is the standard treatm<strong>en</strong>t for colon cancer and remains<br />

the only curative <strong>op</strong>tion.<br />
5.4.2 Primary data selection in MCD 2004<br />
First selection of data was based on ICD-9-CM diagnostic and procedure co<strong>de</strong>s as<br />
178, 179<br />
pr<strong>op</strong>osed by Harmon et al. and Rab<strong>en</strong>eck et al., and on NIHDI procedure co<strong>de</strong>s.<br />
Ne<strong>op</strong>lasms of the rectosigmoid junction (i.e. the segm<strong>en</strong>t of the gut where the colon<br />
merges with the rectum) should also be inclu<strong>de</strong>d in the selection. Therefore, the<br />
Hartmann’s procedure was ad<strong>de</strong>d. This procedure refers to the anterior resection of<br />
the sigmoid and the rectum with closure of the distal resection margin and <strong>en</strong>d<br />
colostomy. According to Vlay<strong>en</strong> et al., Hartmann’s procedure is <strong>en</strong>co<strong>de</strong>d with the non<br />
specific ICD-9-CM procedure co<strong>de</strong> 45.75 (for left hemicolectomy) and the very specific<br />
NIHDI procedure co<strong>de</strong> 244064. 180<br />
This selection resulted in a total of 20 264 stays. This selection is too broad since it also<br />
inclu<strong>de</strong>s pati<strong>en</strong>ts with a secondary diagnosis of malignant ne<strong>op</strong>lasm. This implies that<br />
pati<strong>en</strong>ts with colon cancer as pre-existing condition are also inclu<strong>de</strong>d (see section 4.2.1,<br />
page 39). The <strong>de</strong>finition of the p<strong>op</strong>ulation studied will be <strong>de</strong>fined in the next section.<br />
