Part 4: Treatment and management of ASD Part4 Treatment and management of ASD This section covers the management of behavioural, emotional and mental health difficulties that can be experienced by children, young people and adults with ASD. Many of these difficulties can be prevented, minimised or alleviated by interventions with a totally educational component (see Part 3: Education for learners with ASD). Part 4: Treatment and management of ASD 4.1 Introduction Summary of recommendations Recommendations: 4.1.1 Treatment should encourage functional development, skills for independent living, and minimise stress on the person with ASD and their family and whänau. 4.1.2 Treatments of ASD and co-morbid conditions should be matched to the specific characteristics and needs of the person concerned. 4.1.3 Treatments of ASD and co-morbid conditions should be based on comprehensive diagnostic and ongoing assessments. 4.1.4 Pre-treatment assessments should gather detailed information on behavioural, emotional and mental health difficulties, address differential diagnosis, screen for medical conditions and address environmental issues. 4.1.5 Treatment plans should be comprehensive, and include behavioural needs, educational interventions, psychosocial treatments, communication, environmental and systems issues and the suitability (or not) of medication. Grade B B B B B Autism spectrum disorders are behaviourally defined. Working through the symptoms of ASD, it is clear that behavioural and emotional problems typify the disorder, and that long-term problems may arise if the communicative, social and ritualistic/obsessive aspects of ASD are not attended to 248 258 259 . Common behavioural problems include hyperactivity, attention difficulties, repetitive and ritualistic behaviours, self-injury, tics, and unusually strong interests 34 260 . Problems with mood are common 34 , as are social difficulties 261 . Not surprisingly, parents of children and young people with ASD frequently seek behavioural advice, as do families and carers of those adults with ASD who also have an intellectual disability. Good treatment of ASD encourages normal development and skills for independent living, while minimising the stress of the person with ASD and their family, and treatments are likely to be more successful if they are targeted on specific skills development (Recommendation 4.1.1). There is a growing literature on the mental health concerns of people with ASD. Emotional problems, such as poor anger and anxiety control and depression, appear to be common experiences of young people and adults with ASD 261 262 , especially at times of transition and at adolescence. Major psychiatric disorders (eg, bipolar affective disorder, psychoses, schizophrenia and catatonia) have also been described in research and academic case studies 18 46 262-267 . A study of the diagnostic accuracy of the Autism Spectrum Disorder Screening Adults Questionnaire found that 1.4% of the psychiatric population screened (n=1323) had ASD, most previously undiagnosed 66 . The increased prevalence of psychiatric problems in the population of people with intellectual disabilities has been well established in the literature 159 . Preliminary research on the prevalence of psychiatric symptomatology in people with intellectual disabilities with and without ASD suggests that those with ASD have a much higher chance of developing psychiatric problems 260 .
