<strong>Ph</strong>.D. <strong>Program</strong> <strong>Guidebook</strong> 2007-2008<br />

<strong>The</strong> <strong>University</strong> <strong>of</strong> <strong>Chicago</strong> transcript is a record <strong>of</strong> a student’s progress and performance<br />

while enrolled at the university. All courses completed or withdrawn from for a grade <strong>of</strong> W<br />

appear on the <strong>of</strong>ficial student transcript with the grade received. All entering students pay a<br />

lifetime transcript fee <strong>of</strong> $39 in their first year <strong>of</strong> study. <strong>The</strong> lifetime transcript fee allows<br />

students to request an unlimited number <strong>of</strong> <strong>of</strong>ficial transcripts, now and in the future,<br />

whenever they need it, at no additional cost. Transcripts may be ordered in one <strong>of</strong> four ways:<br />

1. By a secured Internet transaction via registrar.uchicago.edu/transcripts.html.<br />

2. In person in the Office <strong>of</strong> the <strong>University</strong> Registrar.<br />

3. By mail, sending a signed request to the Office <strong>of</strong> the Registrar, 5801 South Ellis<br />

Avenue, Room 103, <strong>Chicago</strong>, IL 60637.<br />

RESTRICTED REGISTRATION<br />

<strong>The</strong> university applies restrictions to students’ university accounts in two categories.<br />

Category One: Penalties are applied locally to students’ accounts by the restricting <strong>of</strong>fice.<br />

Restrictions in this category include:<br />

Office Placing Restriction<br />

Library<br />

Bursar<br />

Reason for Restriction<br />

unpaid fines<br />

returned checks<br />

<strong>The</strong> Registrar’s Office notifies students <strong>of</strong> the penalty and its consequences. Category One<br />

restrictions will not interfere with the student’s continuing registered status and enrollment in<br />

courses.<br />

Category Two: Penalties deny registration privileges and are imposed for three types <strong>of</strong><br />

reasons: academic, financial, or noncompliance with federal or state regulations. Restrictions<br />

in this category include:<br />

International Affairs<br />

Library<br />

Parking Office<br />

Real Estate Operations<br />

Student Care Center<br />

Student Loan Administration<br />

<strong>University</strong> Dean <strong>of</strong> Students<br />

Reason for Restriction<br />

unpaid balances,<br />

personal bankruptcy<br />

check-in required for all new international students<br />

unreturned materials,<br />

unpaid fees<br />

parking tickets<br />

delinquent rent payments<br />

noncompliance with<br />

immunization requirements<br />

no exit interview<br />

disciplinary actions<br />

<strong>The</strong> restricting <strong>of</strong>fice, the <strong>University</strong> Registrar and the <strong>Ph</strong>.D. <strong>Program</strong> Office will<br />

communicate the restrictions to the student.<br />
