<strong>Ph</strong>.D. <strong>Program</strong> <strong>Guidebook</strong> 2007-2008<br />

continuation plan may elect to maintain and pay for health insurance for the duration<br />

<strong>of</strong> the current plan year (August 31). In electing a continuation <strong>of</strong> insurance<br />

coverage, students are assessed the quarterly university Health Services Fee, unless<br />

they will be residing more than 100 miles from campus. Please contact the <strong>Ph</strong>.D.<br />

<strong>Program</strong> Office for further information.<br />

2. Health Services and Activities Fees: Students do not pay these fees while on leave <strong>of</strong><br />

absence. <strong>The</strong> only exception is for students taking an approved medical leave <strong>of</strong><br />

absence or those students participating in the continuation plan. In those instances,<br />

they are required to pay for the health services fee in the same quarters. Students<br />

may not elect to pay for the activities fee while on leave <strong>of</strong> absence; they may not<br />

participate in <strong>Chicago</strong> GSB or university-sponsored events while on inactive status.<br />

Please contact the <strong>Ph</strong>.D. <strong>Program</strong> Office for further information.<br />

3. International students: International students must complete a leave <strong>of</strong> absence form<br />

with the Office <strong>of</strong> International Affairs and inquire about what documents will be<br />

required upon resumption. Taking a leave <strong>of</strong> absence may affect international<br />

students’ eligibility for Curricular Practical Training or Optional Practical Training.<br />

Students should inquire about eligibility requirements at the Office <strong>of</strong> International<br />

Affairs.<br />

<strong>The</strong> following are specific considerations for Armed Forces Reservists:<br />

1. Students should present <strong>of</strong>ficial documentation <strong>of</strong> their status as an Armed Forces<br />

Reservist to the <strong>Ph</strong>.D. <strong>Program</strong> Office.<br />

2. Students receiving VA benefits should speak with the designated Advisor to<br />

Veterans in the Office <strong>of</strong> the Registrar for the <strong>University</strong>.<br />

3. Students called to duty before week five <strong>of</strong> the quarter will be administratively<br />

dropped from their courses without tuition penalty.<br />

4. Students called to duty during week five or thereafter will determine appropriate<br />

registration/grading options on a per course basis.<br />

Resumption <strong>of</strong> Studies after Leave <strong>of</strong> Absence<br />

Students must contact the <strong>Ph</strong>.D. <strong>Program</strong> Office at least six weeks prior to their expected<br />

return in order to ensure they have the maximum amount <strong>of</strong> time to bid for courses.<br />

International students also should contact the Office <strong>of</strong> International Affairs to confirm their<br />

expected return date.<br />

Although readmission to a student’s program is not automatic, a student with a good record<br />

and a good reason for the extended absence is usually permitted to resume, subject to the<br />

time limit for completion <strong>of</strong> the degree.<br />

Inactive Status: Students are placed on inactive status when they take a leave <strong>of</strong> absence.<br />

Inactive students do not have access to the following <strong>Chicago</strong> GSB and university privileges:<br />

• ORB and other password-protected academic information, such as course<br />

evaluations, restricted course materials, and Chalk<br />

• Community Directory, except to update address information<br />

• Financial aid<br />

• Health insurance, except for <strong>Ph</strong>.D.<strong>Program</strong> students participating in the continuation<br />

plan or those taking a leave <strong>of</strong> absence for medical reasons<br />
