Students’ <strong>Chicago</strong> GSB e-mail accounts remain active while on inactive status. After four<br />
quarters <strong>of</strong> inactive status, students are removed from the administrative e-mail distribution<br />
lists. <strong>The</strong>y are automatically added back to these lists upon resuming active status. Access to<br />
password-protected information on the <strong>Chicago</strong> GSB Portal is regained upon resuming active<br />
status.<br />
Students’ university CNetID becomes dormant for students who are not registered for<br />
courses for six or more months. Upon registration in the quarter <strong>of</strong> return, the CNetID is<br />
automatically reactivated. Students regain access to all <strong>Chicago</strong> GSB and university<br />
privileges listed above in the quarter <strong>of</strong> registration.<br />
HEALTH INSURANCE AND STUDENT HEALTH AND WELLNESS FEE<br />
<strong>The</strong> <strong>University</strong> requires <strong>Ph</strong>.D. students to maintain health insurance coverage. Coverage<br />
must be annually maintained by enrolling in one <strong>of</strong> two university Student Accident and<br />
Sickness Insurance (SASI) Plans or waiving enrollment by documenting coverage through<br />
another source. <strong>The</strong> coverage dates <strong>of</strong> the SASI plans are September 1, 2007 through August<br />
31, 2008. Insurance premiums are assessed over three quarters. Students who participate in<br />
the plan during autumn, winter, and spring quarters <strong>of</strong> an academic year are automatically<br />
covered for the subsequent summer quarter without an additional payment. To fulfill this<br />
requirement, students will need to enter the Student Insurance Open Enrollment website<br />
https://registrar.uchicago.edu/health and either elect SASI coverage or apply for a waiver by<br />
demonstrating adequate coverage.<br />
All registered students must pay the Student Health and Wellness Fee. <strong>The</strong> fee cannot be<br />
waived for participants who live within 100 miles <strong>of</strong> the <strong>University</strong> and waive enrollment in<br />
the SASI plan. <strong>The</strong> Fee will be waived ONLY for those students who live and study over<br />
100 miles from campus, and who will not be on campus during the quarter.<br />
IMMUNIZATION REQUIREMENTS<br />
By State <strong>of</strong> Illinois law, all students are required to present pro<strong>of</strong> <strong>of</strong> immunity to rubella<br />
(German measles), measles (rubeola), mumps, and tetanus/diphtheria. <strong>The</strong> Student Care<br />
Center notifies all new students <strong>of</strong> the requirement and provides instructions for<br />
compliance. <strong>The</strong> exact requirements vary for each disease and are different for<br />
international students. Information and immunization forms can be accessed at<br />
scc.uchicago.edu. Students who fail to meet this requirement by the deadline will be<br />
restricted and denied access to <strong>University</strong> facilities and to subsequent course registration.<br />
A student who receives this notification should call the Immunization Office at 773-702-<br />
9975 to resolve their status.<br />

It is a policy <strong>of</strong> the <strong>University</strong> <strong>of</strong> <strong>Chicago</strong> to comply with the Americans with Disabilities<br />
Act and Section 504 <strong>of</strong> the Rehabilitation Act as amended. Students with disabilities,<br />
including learning-related disabilities, which might qualify for academic program<br />
