4.06 Withdrawals - RefundsOfficial withdrawal from the university is dated at the time a student signs a withdrawal request and files it in theRegistrar's Office. Forms for withdrawal may be obtained in the Registrar's Office. The date of official withdrawaldetermines the charges assessed and/or refunded.Non-attendance in classes or moving from the residence hall does not constitute official withdrawal from theuniversity or from a course. Please refer to Section 4regarding payment options.5.02 Application Information - Financial AidStudents interested in applying for financial assistance must be accepted for admission to <strong>Concordia</strong>, submit the FreeApplication for Federal Student Aid (FAFSA), and <strong>Concordia</strong>'s Application for Financial Aid. Church-Work studentsmust also complete a Declaration of Intent from their home church. When all required forms have been received by theOffice of Student Financial Services, the student's eligibility is computed based on a formula arriving at a contributionto be made by the family and the student toward educational costs. An award package is then constructed by the Officeof Student Financial Services through combinations of scholarships, grants, low-interest loans, and on-campus workstudyprograms.Eligibility for all financial assistance (see following list of programs) requires that a student be accepted for admissionor enrolled and maintain satisfactory academic progress according to the standards for Satisfactory Academic Progressfor Financial Assistance as listed.
