<strong>BNF</strong>C <strong>2011</strong>–<strong>2012</strong> 7.3.4 Contraceptive devices 407should provide full counselling, backed, where available,by the patient in<strong>for</strong>mation leaflet. Devices shouldnot be fitted during the heavy days of the period; theyare best fitted after the end of menstruation and be<strong>for</strong>ethe calculated time of implantation. The main excessrisk of infection occurs in the first 20 days after insertionand is believed to be related to existing carriage of asexually transmitted infection. Women under 25 yearsare at a higher risk of sexually transmitted infections,and pre-insertion screening (<strong>for</strong> chlamydia, and dependingon sexual history and local prevalence of disease,Neisseria gonorrhoeae) should be per<strong>for</strong>med. If resultsare unavailable at the time of fitting an intra-uterinedevice <strong>for</strong> emergency contraception, appropriate prophylacticantibacterial cover should be given. Thewoman should be advised to attend as an emergencyif she experiences sustained pain during the next 20days.An intra-uterine device should not be removed in midcycleunless an additional contraceptive was used <strong>for</strong>the previous 7 days. If removal is essential post-coitalcontraception should be considered.If an intra-uterine device fails and the woman wishes tocontinue to full-term the device should be removed inthe first trimester if possible.INTRA-UTERINE CONTRACEPTIVEDEVICESCautions see notes above;
