Chapter 3: Getting StartedEQAS Method GuideThe EQAS Method Guide is provided on a compact disc (CD). Registered EQAS participants can also accessthe latest version on www.QCNet.com. The EQAS Method Guide contains the most commonly usedmethodologies and instruments currently in use by laboratories worldwide.The EQAS Method Guide is updated regularly to ensure Bio-Rad EQAS Programs are current with the latesttechnologies used in clinical laboratories. If you have a method, instrument, reagent, or unit not shown in theEQAS Method Guide, contact your local Bio-Rad office or representative for assistance.The EQAS Method Guide contains the following sections:• Searching Codes by Popular InstrumentUse this section of the of the EQAS Method Guide for help with selecting the proper method and to helpminimize coding errors.Important: Some instruments can be used with different reagents for the same analyte and cantherefore be listed under different methodologies in the EQAS Method Guide. Make sure to use thecorrect method code for the methodology associated with the reagent you are using.• Searching Codes by AnalyteThis section of the EQAS Method Guide contains codes for reportable analytes with their applicablemethods and units.• Instrument/Kit CodesThis section of the EQAS Method Guide contains codes for most instruments/kits.• Reagent CodeThis section of the EQAS Method Guide contains codes for most reagents.• Special Section for Selected Coagulation TestsUse this section of the EQAS Method Guide for help with selecting the proper method, instrument,reagent, and unit codes when reporting Prothrombin Time (PT/INR) and Activated Partial ThromboplastinTime (APTT). Chapter 3: Getting StartedEQAS Documentation (Document Pack: For Paper Customers Only)A Document Pack is sent to paper customers after Bio-Rad receives the Method Registration Form. TheDocument Pack is mailed separately from the Specimen Package and consists of the following items for agiven EQAS Program cycle:• Cover Page• Laboratory Profile Form• Registration Guide• Results Forms• Blank Results Forms• Change Notification FormsCover PageThe cover page contains three sections:• Instructions for submitting your results to Bio-Rad.• Important EQAS news or announcements for the program and cycle.• Your lab number and mailing address.Please review your address information for accuracy. Use the Laboratory Profile Form if you need any changesmade to your laboratory information (see below).Laboratory Profile FormThe Laboratory Profile Form shows your laboratory demographic information on file with Bio-Rad EQAS.Please review this information. If any information is inaccurate or incomplete, use the “Changes Required”column and submit the updated Laboratory Profile Form to Bio-Rad. Make sure the Laboratory/Hospital Nameand Department information are as you want them to appear on your Certificate of Achievement. We willupdate your information upon receipt.Registration GuideThis document shows all tests currently registered to your laboratory. The information is organized byinstrument and the analytes registered to that instrument. Please review each registration to make sure it iscorrect. If a test requires a change to the instrument, methodology, reagent or unit, use the ChangeNotification Form to submit the change. (See “Change Notification Form (Method Changes)” on page 27 formore information.)
