Chapter 3: Getting StartedVerify the Contents of the Document PackPlease take time to verify the contents of the Document Pack when you receive it. All pages should containyour lab number. The program cycle and sample number should be indicated on each pre-printed ResultsForm. If you find any errors in your Document Pack, contact your local Bio-Rad office or representative, orsend an email to eqas@bio-rad.com.If your laboratory prefers to use <strong>EQAS</strong> Online, <strong>EQAS</strong> Mobile, or the <strong>EQAS</strong> eForms 2 software, you will notreceive a Document Pack. These environmentally friendly options ( ) are the preferred means for submittingyour <strong>EQAS</strong> results and method registrations to Bio-Rad.Specimen PackageImportant: Make sure you read the package insert included in the specimen package. Thepackage insert includes information on the proper storage, handling, and reconstitution (ifapplicable) of the samples, and describes any limitations in the time you may perform selectedtests.The specimen package is sent to you separate from your Document Pack. You will receive a specimenpackage for each program cycle you enroll in and submit an order for. The specimen package contains all thesamples to be tested in one <strong>EQAS</strong> cycle. The specimen package consists of the following items:• Twelve (12) vials of freeze-dried serum, urine, or human whole blood (see note below).• Preparation of select samples are subcontracted to qualified suppliers. The specimen package, samplevial, and package insert will be clearly marked withwhere applicable.• Each vial is labeled with a specific sample number (1–12).• Each vial is labeled with the sample due date, cycle number, and lot number.Note: The specimen packages for one cycle of the <strong>EQAS</strong> Hematology <strong>Program</strong> will be sent to youin multiple (four) shipments. Each shipment contains three liquid samples.Verify the Contents of the Specimen PackageMake sure you inspect the specimen package upon receipt to ensure it is complete and undamaged. The vialsare clearly identified by Sample Number. There should be one numbered vial for each cycle sample. Inspectthe vials for any damage that may have occurred during their transport. Inspect samples associated with theHematology <strong>Program</strong> for hemolysis. Notify your local Bio-Rad office or representative if you have any concernabout the specimen package.
