CHAPTER8Frequently Asked QuestionsQuestion:Answer:Can I join an <strong>EQAS</strong> <strong>Program</strong> after a cycle has started?Yes, however it is subject to the availability of inventory. Follow the usual registrationprocedure. Complete the Order Form and Method Registration Form (if applicable). As soonas you receive your Document Pack or by using an electronic reporting option, assaysamples that are already beyond the Sample Due Dates. If submitting results by paper, makesure you use the appropriately dated Results Forms and return to Bio-Rad. Retrospectivereports showing your results will be created on request.Question:Answer:What if I make an error in reconstituting the sample?Contact your local Bio-Rad representative. Bio-Rad can supply another vial for you to assay(subject to available inventory). Full instructions on reconstitution and storage are described inthe package insert that comes with the product.Question:Answer:Are the samples animal or human-based?The Clinical Chemistry, Immunoassay, and Therapeutic Drug Monitoring programs’ sera arehuman-based. Human urine is used in the <strong>EQAS</strong> Urine Chemistry <strong>Program</strong>. The whole bloodprovided in the Hemoglobin <strong>Program</strong> is human based. The Hematology <strong>Program</strong> iscomprised of both human and non-human components. If submitting by paper, complete aChange Notification Form. Submit the Change Notificationapproximately two weeks prior to the proposed change, if possible.
