Chapter 4: Submit Your EQAS ResultsComplete the Results Form (For Paper Customers Only)Your Document Pack contains 12 Results Forms, one for each sample in the cycle. Make sure you submitresults on the correct form for the sample number tested.The test results should be clearly entered in the correct boxes of the Results Form using the pre-printeddecimal point (identified as a period), as appropriate. For each listed analyte, you can either enter a numericresult or an Unreportable Result (UR) Code. In submitting either result, it is preferable that you write allnumerals in the following format so that your results are more accurately and easily read: 0 1 2 3 4 5 6 7 8 9.Please take special care with 3, 5 and 8, and 6 and 9, where misinterpretation of handwriting can easily occur.Please note that values of zero, less than (), cannot be accepted in the results boxes (see“Unreportable Results (UR) Codes” on page 33). If you want to remove a test (no longer report a result basedon that test configuration), select the applicable “Delete Test” check box. Please note that a deleted test willbe removed from your sample report and all future sample reports.Important: If you have already submitted a Change Notification Form requesting a revision of theinstrument, method, reagent, or unit associated with the test in your laboratory-specific ResultsForms, do not check the “Delete Test” check box unless you want to remove the new testconfiguration or completely remove testing of the analyte in future samples.Unreportable Result (UR) Codes show the possible reasons why data cannot be submitted. Unreportableresults cannot be compared and will not be included in the calculations. The use of this code indicates thatyou attempted to test the sample, but were unable to submit a valid result. These codes can help Bio-RadEQAS to modify analyte targets appropriately in subsequent cycles. Enter the code for your unreportable resultin the UR Code box for the test. Do not enter a UR Code in the “Result” field.If your preprinted laboratory-specific Results Form no longer correlates with the instrument used in yourlaboratory and you did not submit a Change Notification Form, you can use the Blank Results Form to submityour sample results along with your new test coding. The coding submitted in the Blank Results Form willchange your test configuration in the Bio-Rad EQAS database. This new configuration will be applied to allfuture samples regardless of the configuration that was in place at the start of the cycle, which is noted in thelaboratory specific Results Forms.The Blank Results Form is located in your Document Pack and on www.QCNet.com. Amendments forpreviously submitted test results can also be submitted on the Blank Results Form. Make sure that you notethe proper sample number for which test results are being submitted. When there is a change to your testconfiguration either through a Change Notification Form or Blank Results Form, you can request that a newDocument Pack be sent to you through email.Note: This is only possible if a current email address is on file at Bio-Rad EQAS (as listed in yourLaboratory Profile Form).32 EQAS Program User Guide
