Chapter 8: Frequently Asked QuestionsQuestion:Answer:Some assays are carried out infrequently in my laboratory. Can I submit results after theSample Due Date?Yes. If submitting by paper, complete an appropriately dated Results Form. If you havealready used the preprinted form, use one of the Blank Results Forms provided in yourDocument Pack or under the <strong>EQAS</strong> section of www.<strong>QCNet</strong>.com. If submitting results via<strong>EQAS</strong> eForms 2, <strong>EQAS</strong> Online, or <strong>EQAS</strong> Mobile, you may do so at anytime within the cycle(and within the sample expiration date). A retrospective, updated report can be supplied onrequest. Late results will be indicated on your report.Question:Answer:Can I submit data electronically?Yes. The <strong>EQAS</strong> eForms 2 software is provided free of charge to all participants. You can sendyour results via email (Export and Send file) or upload your results (Export file) online atwww.<strong>QCNet</strong>.com. Bio-Rad also offers a free of charge <strong>EQAS</strong> Online option available onwww.<strong>QCNet</strong>.com. Online reporting is also available for all smartphones (<strong>EQAS</strong> Mobile).Question:Answer:What if I require more of the sample for testing?You can purchase additional specimen packages. If you know in advance you will need moresample than is provided in the specimen package, indicate your requirement on the OrderForm. Additional specimen packs are identified with the prefix “QC.”Question:Answer:Do you have a customer complaint procedure?We hope you do not have cause to complain. If this occurs, please contact our CustomerSupport by letter, fax, email, or telephone as soon as possible so that we can act promptly.All complaints are logged into a database and given a reference number.
