110Second‐level physical defect case during 2004‐2007 was relatively stable(8.6%‐8.7%), but in 2008‐2010 it gradually increased to 10.79% (in 2010), asshown in Table 4.3. Proporsion of second‐level physical defect by 2010 had notmet the target of < 5%. Filariasis ControlFilariasis (usually called as elephantiasis) spreads almost thoughoutprovinces in Indonesia. Filariasis elimination program is established based onWHO Global 2000 “The Global Goal of Elimination of Lymphatic Filariasis as aPublic Health Problem the year 2020” as realization of WHA (World HealthAssembly) resolution in 1997.This elimination program is implemented through 2 pillars:1. Mass Drug Administration (MDA) to all people in filariasis endemic districtsusing DEC 6 mg/kg BB combined with Albendazole 400 mg once a year for 5years in order to cut the transmission.2. Case management of filariasis clinical cases to prevent and reduce physicaldefect.Since 2005, Implementation Unit (IU) used in Filariasis EliminationProgram is district/municipality. It means the smallest area in this program isdistrict/municipality, both for endemicity determination and mass drugadministration. When a district/municipality has been a filariasis endemic, targetof mass drug administration covers all residents of the district/municipality. Theyhave to take medicine, but it is temporarily delayed for children under‐two (2)years, pregnant woman, people with serious illness, filariasis chronic patient inacute attack and marasmus/kwashiorkor under‐five.
