2009 <strong>CityMatCH</strong> Urban MCH Leadership <strong>Conference</strong>ACCOMPLISHMENTS/RESULTSInitial outcomes for <strong>the</strong> Nurse-Family Partnership and Empowering Families of Milwaukee programshave indicated that <strong>the</strong> percentage of preterm or low birth-weight babies of clients are half <strong>the</strong> rate of thatof <strong>the</strong> zip codes in which <strong>the</strong>y provide service.Evaluation Plan:In 2009, all four programs are syn<strong>the</strong>sizing <strong>the</strong>ir evaluation plans to assure that <strong>the</strong> same indicators aremeasured. Data sets include process indicators such as <strong>the</strong> number of visits conducted, as well asoutcomes indicators such as: rate of low-birth weight babies, percent of clients who quit smoking, percentof clients who sustained breast-feeding, and percentage of children reaching age-appropriatedevelopmental milestones.BARRIERSThe major barrier is that clients are harder to reach: young, at-risk pregnant women do not always accesshealth care services early in <strong>the</strong>ir pregnancy; more women are working outside <strong>the</strong> home; and distrust ofanyone who might want to visit <strong>the</strong>m at home makes it difficult to establish an initial relationship with <strong>the</strong>client.LESSONS LEARNEDThere is no “one size fits all” approach to home visiting. Ra<strong>the</strong>r than attempt to create one monolithicprogram that can meet <strong>the</strong> needs of all, <strong>the</strong> MHD encouraged <strong>the</strong> organic development of separate butconnected home visiting programs. Staff can be trained in <strong>the</strong>ir specific program model, allowing forgreater depth of skills and knowledge. And each referral received by <strong>the</strong> health department is sure to be agood match for one of <strong>the</strong> programs allowing clients to receive comprehensive care that is tailored to <strong>the</strong>irunique circumstances.123
