2009 <strong>CityMatCH</strong> Urban MCH Leadership <strong>Conference</strong>• Effective engagement of <strong>the</strong> entire community in identifying strategies to assure optimal health for <strong>the</strong>MCH population.OHS provides in-home and office visitation, nursing assessments, health education and training, supportgroup meetings, leadership development training, male involvement services, and transportation servicesto support <strong>the</strong> active participation in its HealthNET case management program.Annually, OHS interfaces with 250+ pregnant and postpartum, women, and 300+ infants and toddlers 0 to2 years of age. In addition, OHS works one-on-one with 150 men through its male involvement initiative.Efforts are made to integrate <strong>the</strong> on-site health care services provided by CDHC on behalf of all OHSprogram participants.OHS is working to develop an interdisciplinary review panel that gives oversight for <strong>the</strong> implementationof its life course model. The proposed abstract presentation will exam <strong>the</strong> overall success of implementing<strong>the</strong> model; <strong>the</strong> processes used to identify OHS program participants for inclusion in this initiative;integrating existing TQM policies and procedures in <strong>the</strong> overall review process; development of toolsneeded to track participant compliance, service delivery, interdisciplinary team meetings andrecommendations; and identification of barriers and challenges in successfully implementing thisinitiative.It is imperative that <strong>the</strong> OHS team identify “champions” for <strong>the</strong> development and implementation of thisinitiative. OHS is poised to work with <strong>the</strong> State of Nebraska Office of Women’s Health to develop aneffective process for disseminating findings associated with this initiative.46
