Each year, <strong>the</strong>re are asignificant number of deaths due to unsafe sleeping. The Cribs for Kids program was identified as apossible program to decrease deaths due to unsafe sleep.SETTINGThis program targets all of <strong>the</strong> birthing hospitals in <strong>the</strong> city, <strong>the</strong> Bureau of Child Welfare, <strong>the</strong> City ofMilwaukee Health Department (MHD), <strong>the</strong> Milwaukee Fire Department, and o<strong>the</strong>r community agenciesas part of <strong>the</strong> program.PROJECTPhase I: included two of <strong>the</strong> birthing hospitals who have approximately 55% of <strong>the</strong> births in Milwaukee.One of <strong>the</strong> hospitals identified <strong>the</strong> Cribs for Kids program and <strong>the</strong> use of Pack ‘n Plays ® to provide safesleep. This institution created guidelines and teaching strategies to use when identifying a family whoneeds a safe place for <strong>the</strong>ir infant to sleep. Funding was secured <strong>from</strong> a foundation at <strong>the</strong> hospital andthrough a local grant. A safe sleep “champion” organized <strong>the</strong> delivery of <strong>the</strong> program at <strong>the</strong> hospital. Oneyear later, ano<strong>the</strong>r large hospital identified funds to use for Pack ‘n Plays ® and modeled <strong>the</strong>ir programafter <strong>the</strong> first one.Phase II: involved <strong>the</strong> MHD, who identified at-risk infants through <strong>the</strong>ir home visiting programs and <strong>the</strong>Milwaukee Fire Department, who identified at-risk infants when making an Emergency Medical Servicecall.Phase III: included a large clinic in Milwaukee that serves an at-risk population, several HMOs and <strong>the</strong>Bureau of Child Welfare.• In many communities, <strong>the</strong> Cribs for Kids program issues Pack ‘n Plays ® through one centralizedcommunity location.
