In USD 500, 73% of students come <strong>from</strong> households below <strong>the</strong> federalpoverty level.PROJECTThe project model consists of three main components: screening, follow-up, and referral.• Screening: The Hip Hop for Health Fairs take place at 14 USD 500 middle and high schools. Students<strong>from</strong> physical education classes assemble in <strong>the</strong> gymnasium for introductory presentations. Students <strong>the</strong>nvisit interactive exhibit stations to learn about various health topics and community resources. Theatmosphere is fast-paced, fun and educational. Selected popular hip hop music is played in <strong>the</strong>background. AYS and UGPHD collaborate to screen students for height, weight, body mass index (BMI),and blood pressure.• Follow-up: UGPHD and AYS collaborate to contact youth and <strong>the</strong>ir parents through mail and phonecalls. Written information is provided about BMI screening results and <strong>the</strong> possible health and socialconsequences that could result <strong>from</strong> being overweight or obese.• Referral: Parents are encouraged to discuss <strong>the</strong> screening results with <strong>the</strong>ir family health care provider.Parents are provided with contact information for weight management clinics available at Kansas132
