Champions for Inclusive Communities (ChampionsInC.org), funded through the U.S. MCHB,serves as a research and development support center for state level MCH/CYSHCN programs. State andlocal communities across the country are working with us in building systems of care and support forCYSHCN. Our data document the process of coalition building and the effectiveness of communitycoalitions in this systems building effort as well as the resource they provide to families. Coalitions asprivate/public partners (including parents and youth) have demonstrated the effectiveness of coalitionbuilding for this population. It cross-cuts public and private services/parent-providerpartnerships/pediatric medical homes, educational settings and inclusion in the life of the community inwhich these children and their families live.In this presentation, case studies of communities of excellence will be highlighted and used as models forsystems building efforts. We are in the process of selecting at least one site to co-present this work withmore specifics on how they work together and problem solve the issues they face. The communities to behighlighted represent a range of ethnicity, size, resources and geography. The individual efforts arefunded in a variety of ways and represent the full spectrum of ethnicity, size, collaboration andeffectiveness.If accepted, depending upon the amount of time our session will be provided, we have selected coalitionsfrom several different locales to present with us. 2009 CityMatCH Urban MCH Leadership Conferencehow coalition-building forms strong ties and builds better safety nets for children and families withspecial health care needs. In addition to the relationships that are fostered, concrete outcomes can bedemonstrated as a function of coalition activities. Many of the coalitions we are following use a PARapproach in problem-solving. In general they have better family-provider partnerships, are able to be moreflexible and resilient in changing circumstances and have at least rudimentary continuous qualityimprovement processes to guide their work.INFORMATION FOR REPLICATIONWe are in the process of doing a cost study as part of our work with several communities. We will be ableto present preliminary data on costs and budgets by the time of the conference69
