Relying on <strong>the</strong> multidisciplinary teams consisting of outreach workers, nurses, social workersand various specialists, program staff develops sound strategies with <strong>the</strong> assistance of best modelpractices, data abstraction and evaluation to address areas of concern in an identified project area. Byusing portable computers, Healthy Start, Inc. can capture <strong>the</strong> data accurately, save on printing costs, andevaluate <strong>the</strong> data accurately.OBJECTIVESTo provide audience members with an outline on how to simplify data collection processes through <strong>the</strong>use of new technology available to home visiting programs, and to provide a description of <strong>the</strong> HealthyStart, Inc. model of utilizing data collection and abstraction on health and social issues disparatelyeffecting women of color in targeted communities resulting in specific outreach and case managementstrategies to promote positive outcomes for women and <strong>the</strong>ir families.METHODSAudience will receive specific examples of <strong>the</strong> Participant Information Management System used byHealthy Start including an interactive demonstration, as well as program strategies and outcomes resulting<strong>from</strong> data analysis, both quantitative and qualitative. Audience will also receive examples of products/resources used to coordinate care for consumers including male-based programming, depression screeningtools, database equipment and evaluation reports.81
