Operationalizing this approach offers a challenge for planning and delivering maternaland child health (MCH) services at <strong>the</strong> community level.SETTINGAs one of 30 state-designated, community MCH organizations, <strong>the</strong> Nor<strong>the</strong>ast Florida Healthy StartCoalition, Inc. is responsible for developing a five-year service delivery plan and annual action plan toguide funding and programmatic decisions related to Florida’s Healthy Start initiative. The servicedelivery plan also serves as <strong>the</strong> local health systems action plan (LHSAP) for its federal Healthy Startprogram, <strong>the</strong> Magnolia Project. In past planning efforts, <strong>the</strong> Coalition utilized assessment tools, such as<strong>the</strong> Perinatal Periods of Risk (PPOR), that moved beyond <strong>the</strong> prenatal period. As a result, increasedattention and resources have been directed to preconception health and broader social determinants in itsprogram strategies. For its 2009-2014 service delivery plan, <strong>the</strong> Coalition recognized <strong>the</strong> need to utilize abroader, life-course perspective.PROJECTA life-course framework was developed for conducting <strong>the</strong> five-year needs assessment and planningprocess. This framework offered an alternative approach to examining birth outcomes and healthdisparities that considered health and development over <strong>the</strong> mo<strong>the</strong>rs’/families’ life course, not just duringpregnancy. It also broadened <strong>the</strong> focus of <strong>the</strong> planning process to include health and social equity.A community Planning Committee examined data on MCH health status, available health and socialservices, and social determinants (economic, educational and environmental) for four phases of <strong>the</strong>18
