2009 CityMatCH Urban MCH Leadership Conferencedelivery.ACCOMPLISHMENTS/RESULTS• Using known social determinants of health, a culturally-appropriate EFI tool has been adapted to fit theEFM population.• All EFM staff have been trained in the EFI history and protocol, proper equipment use, and effectiveinterviewing techniques.• Full implementation is underway and preliminary data may demonstrate:- Improved psychosocial markers and an increased attainment of identified care plan goalsthroughout duration of project.- Increased utilization of services and resources.Evaluation Plan:The EFI project will be able to analyze the qualitative data obtained through the longitudinal interviewsand identify strengths and barriers that affect family functioning. In addition, the following quantitativeoutcome indicators will also be analyzed:• The number of services or resources provided to the family based on their interview content, and thenumber of these resources that are used by the family.• Care plan goal attainment.• Participants’ psychosocial and developmental markers.A final report will outline best practices for providing care to high-risk families to improve birthoutcomes and healthy child development.BARRIERSOverall project completion and analysis may be affected by staff turnover and program attrition over thethree-year span of the project.LESSONS LEARNEDHistorically, home visiting assessments are scripted question and answer formats. Introducing a guidedconversation, with open-ended questions and no finite answers has been an innovative paradigm shift inthe way that many home visitors have practiced. Incorporating training, mentoring, and consultation intothe implementation of the project will be a key to successful outcomes.50
