Ininvestigating this issue, the City of Milwaukee Health Department (MHD) discovered that patients inhospital birthing units were receiving conflicting information about safe sleep. Lactation consultants’advice is sometimes at odds with expert-recommended safe sleep practices. Literature indicates thatparents do follow practitioners’ advice, but they are often confused by fractured or conflicting messages.SETTINGThe Wisconsin Association for Perinatal Care (WAPC) has a group in each region of the state. An MHDpublic health nurse, who is also a representative from the WAPC Southeastern region, called an initialmeeting to discuss the need to promote breastfeeding and teach safe sleep using a consistent message.Members of the Southeastern region of WAPC, and other community partners, were invited to themeeting.PROJECTThe goal of this program was to bring the birthing units from local area hospitals together and make acommitment for safe sleep to be taught by all staff, as a strategy to reduce the incidence of SIDS deaths.During the first meeting, the group laid the foundation for addressing the issue. A presentation includeddiffering viewpoints on bed-sharing as a way to improve success in breastfeeding and also as a risk forincreased deaths due to SIDS. This first meeting lead to a number of meetings over a one year period,during which participants discussed policies on breastfeeding and safe sleep, such as the Academy ofBreastfeeding Medicine and the American Academy of Pediatrics policy on the reduction of SIDS.142
2009 CityMatCH Urban MCH Leadership ConferenceRESULTS• To provide safe sleeping for an infant, teaching on each birthing unit needed to foster room sharing, notbed sharing, even when mother was breastfeeding.• The message needs to be the same whether it was delivered by a lactation consultant or a nurse.• Each unit agreed to use the Continuing Education Program on SIDS Risk Reduction: Curriculum forNurses” as a mandatory teaching module for the nurses. Each hospital agreed to implement the curriculumby June 2009.In addition, a poem was created by a member of the group to be used in the bed of every newborn thatoutlines strategies to provide a safe sleep place for their new baby. The infant cards will be printed andgiven to the hospitals to use with each birth.Full implementation of these suggestions will take place through WAPC and hopefully expand to the restof the state.BARRIERSIt is difficult to get multiple practitioners to teach materials the same way.LESSONS• On controversial issues, there is a need to present all sides of the issue, allow for brainstorming, andthen move forward.• Many minds often come up with the best solution.• Health departments are often looked upon as neutral parties, and thus have the ability to call together agroup of competitors and facilitate solutions to difficult problems.143
