General Information 127Eligibility ChangesWithdrawal: A student who withdraws from the <strong>University</strong> during the first ten days of theterm will be refunded the premium fee paid for YHP Hospitalization/Specialty Coverageand/or YHP Prescription Plus Coverage. The student will not be eligible for anyYHP benefits, and the student’s YHP membership will be terminated retroactive to thebeginning of the term. The medical record will be reviewed, and any services renderedand/or claims paid will be billed to the student on a fee-for-service basis. At all othertimes, a student who withdraws from the <strong>University</strong> will be covered by YHP for thirtydays following the date of withdrawal or to the last day of the term, whichever comesfirst. Premiums will not be prorated. Students who withdraw are not eligible to enroll inYHP Student Affiliate Coverage.Leaves of Absence: Students who are granted leaves of absence are eligible to purchaseYHP Student Affiliate Coverage during the term(s) of the leave. If the leave occursduring the term, YHP Hospitalization/Specialty Coverage will end on the date the leaveis granted and students may enroll in YHP Student Affiliate Coverage. Students mustenroll in Affiliate Coverage prior to the beginning of the term during which the leave istaken or within thirty days of the start of the leave. Coverage is not automatic and enrollmentforms are available at the YHP Member Services Department or can be downloadedfrom the YUHS Web site (http://www.yale.edu/uhs/).Extended Study or Reduced Tuition: Students who are granted extended study status or payless than half tuition are not eligible for YHP Hospitalization/Specialty Coverage andYHP Prescription Plus Coverage. They may purchase YHP Student Affiliate Coverageduring the term(s) of extended study. This plan includes coverage for YHP Basic and forthe benefits offered under YHP Hospitalization/Specialty Coverage. Coverage is notautomatic and enrollment forms are available at the YHP Member Services Departmentor can be downloaded from the YUHS Web site (http://www.yale.edu/uhs/). Studentsmust complete an enrollment application for the plan prior to the start of the term.For a full description of the services and benefits provided by YHP, please refer tothe YHP Student Handbook, available from the YHP Member Services Department,203.432.0246, 17 Hillhouse Avenue, PO Box 208237, New Haven ct 06520-8237.Required ImmunizationsMeasles (Rubeola) and German Measles: All students who were born after December 31,1956, are required to provide proof of immunization against measles (rubeola) andGerman measles (rubella). Connecticut state law requires two doses of measles vaccine.The first dose must have been given after January 1, 1969, and after the student’s firstbirthday. The second dose must have been given after January 1, 1980. These doses mustbe at least 30 days apart.
