BarcelonaResumenSe presentan B casos de adenoma hipofisario nosecretante, cuya primera manifestación neurológicaconsistió en un cuadro de apoplejía pituitaria (AP). taintervención quirúrgica practicada en 5 casos permitióobservar cambios necróticos y hemorrágicos entre losrestos tumorales. En los tres restantes se asistió a unaevolución espontánea favorable, con recalcificación delmarco selar en dos de ellos o evolución a silla turcavacía. De estas observaciones se deduce que laindicación quirúrgica no es absoluta en presencia deuna AP, debiendo basarse la conducta terapéutica encriterios clínicos y neurorradiológicos.Palabras clave:Aploplejía p¡tu¡taria. Study of B casesEight cases of non-secreting pituitary adenoma,which first neurologic evidence was a picture ofpituitary stroke (PS) is presented. Necrotic andhemorrhagic changes in the tumour tissue wereobserved in all five cases that were operated on. Theremaining 3 cases showed a favourable clinical course.ln two of them recalcification of the sellar frame orevolution to empty sella turcica was seen. From theseobservations it can be drawn that surgery is not theonly possible treatment of PS, and that the therapeuticapproach should be based on clinical andneuro-radiological criteria.Key words:Pituitary stroke. Pituitary adenoma. Complications.La apoplejía pituitaria (AP) es un cuadro clínicocaracterizado por cefalea de comienzo brusco, síndromemeníngeo, disminución de la agudeza visualy oftalmoplejía. Está causada por el infarto isquémicoo hemorrágico de un adenoma de hipófisis.Puede aparecer como complicación evolutiva deun tumor pituitario ya diagnosticado o bien representarsu primera manifestación clínica. Su conocimientoes de gran interés, ya que puede suscitarproblemas de diagnóstico diferencial con procesosde elevada frecuencia y gravedad, como la meningitispurulenta o la hemorragia subaracnoidea. Depresentación infrecuente, se están realizando nuevasaportaciones sobre su fisiopatología y su historianatural. Es clásico considerar la AP como unaurgencia neuroquirúrgica. En la actualidad, no obstante,se admite Ia posibilidad de una evoluciónespontánea favorable, habiéndose comprobado inclusola curación del propio adenoma hipofisariodespués de un episodio de AP. Los criterios deinclusión en el presente estudio fueron: a/ la apariciónde un cuadro clínico compatible con AP; b/ la demostraciónneurorradiológica de tumor intraselar; c/ la ausenciade un diagnóstico previo de adenoma hipofisario, y d/ lapresencia de cambios necróticos o hemorrágicos en elseno de un adenoma hipofisario probado por cirugía(5 casos) o por TAC (3 casos). A todos los pacientes se lesrealizó una historia clínica y exploración física completas,así como un examen neurológico y oftalmológico detallado(incluyendo agudeza visual, campos visuales y fondode ojo). Se practicó un estudio de rutina bioquímicoelemental, hematológico y de orina, un electrocardiogramay una radiografía de tórax. EI estudio hormonal consistió,en 4 pacientes, en la determinación de 17 hidroxi ycetosteroides urinarios, mientras que en el resto se deter-24
