Par conséquent, la parité zéro <strong>de</strong>s femmes actuellement mariées <strong>et</strong> âgées <strong>de</strong> 35-49 ans, âges auxquels la probabilité d'avoir un premier enfant <strong>de</strong>vient très faible, perm<strong>et</strong> d'estimer le niveau <strong>de</strong> la stérilité totale ou primaire. Parmi ces femmes, entre 2 <strong>et</strong> 4 % n'ont jamais eu d'enfants <strong>et</strong> peuvent être considérées comme stériles. Chez les femmes <strong>de</strong> 45-49 ans dont la probabilité d’avoir un premier enfant est pratiquement nulle, le niveau <strong>de</strong> stérilité primaire est voisin <strong>de</strong> 2 % (2,2 %), chiffre cohérent avec les résultats <strong>de</strong>s enquêtes précé<strong>de</strong>ntes : 1,7 % en 1997 (EDS-III), 2,4 % en 1993 (EDS-II). Ce faible niveau <strong>de</strong> stérilité primaire est également proche <strong>de</strong> celui observé lors d'enquêtes EDS effectuées dans d'autres pays d'Afrique comme le Mali (2 %) <strong>et</strong> la Côte d'Ivoire (2 %). Enfin, le tableau 4.5 présente le nombre moyen d’enfants vivants <strong>et</strong> survivants. Dans l’ensemble, une femme a, en moyenne, 2,7 enfants vivants <strong>et</strong> parmi les femmes en union ce nombre moyen est <strong>de</strong> 3,7, soit 85 % <strong>de</strong>s enfants nés vivants. Autrement dit, 15 % <strong>de</strong>s enfants nés vivants sont décédés ; ceci donne une première idée sur le niveau <strong>de</strong> la mortalité. 4.4 INTERVALLE INTERGÉNÉSIQUE La durée <strong>de</strong> l'intervalle qui sépare la naissance d'un enfant <strong>de</strong> la naissance précé<strong>de</strong>nte a une influence sur l'état <strong>de</strong> santé <strong>de</strong> la mère <strong>et</strong> <strong>de</strong> l'enfant. Son étu<strong>de</strong> revêt par ailleurs une importance certaine dans l'analyse <strong>de</strong> la fécondité. Il est admis que les intervalles intergénésiques courts (inférieurs à 24 mois) sont nuisibles à la santé <strong>et</strong> à l'état nutritionnel <strong>de</strong>s enfants <strong>et</strong> augmentent les risques <strong>de</strong> décès <strong>de</strong> la mère <strong>et</strong> <strong>de</strong> l’enfant. Les naissances trop rapprochées amoindrissent la capacité physiologique <strong>de</strong> la femme ; ce faisant, elles exposent la mère aux complications pendant <strong>et</strong> après la grossesse (fausses couches, Fécondité | 61
éclampsie). Le tableau 4.6 présente la répartition <strong>de</strong>s naissances <strong>de</strong>s cinq années ayant précédé l'enquête selon le nombre <strong>de</strong> mois écoulés <strong>de</strong>puis la naissance précé<strong>de</strong>nte, en fonction <strong>de</strong>s caractéristiques sociodémographiques. On constate que le pourcentage <strong>de</strong>s naissances survenues moins <strong>de</strong> 18 mois après la naissance précé<strong>de</strong>nte est relativement faible : 7 %. Environ, 13 % <strong>de</strong>s enfants sont nés entre 18 <strong>et</strong> 24 mois après leur aîné, soit au total, 19 % <strong>de</strong> naissances espacées <strong>de</strong> moins <strong>de</strong> 24 mois <strong>de</strong> la naissance précé<strong>de</strong>nte (tableau 4.6). Néanmoins, une forte proportion <strong>de</strong>s naissances (38 %) se produisent entre 2 <strong>et</strong> 3 ans après la naissance précé<strong>de</strong>nte, <strong>et</strong> 42 % <strong>de</strong>s enfants sont nés trois ans ou plus après leur aîné. La durée médiane <strong>de</strong> l'intervalle intergénésique est proche <strong>de</strong> 3 ans (33 mois), ce qui signifie que la moitié <strong>de</strong>s naissances intervient dans un intervalle <strong>de</strong> près <strong>de</strong> 3 ans après la naissance précé<strong>de</strong>nte. C<strong>et</strong> intervalle médian est i<strong>de</strong>ntique à celui estimé en 1997 (33 mois selon l'EDS-III). Il en est <strong>de</strong> même pour la proportion <strong>de</strong> naissances se produisant dans un intervalle inférieur à 24 mois par rapport à la naissance précé<strong>de</strong>nte (18 %). L'âge <strong>de</strong>s mères influe sur la durée d'espacement <strong>de</strong>s naissances. En eff<strong>et</strong>, on remarque que les intervalles intergénésiques sont plus courts chez les jeunes femmes que chez les plus âgées : <strong>de</strong> 25,6 mois chez les femmes <strong>de</strong> 15-19 ans, il passe <strong>de</strong> 31,3 mois chez celles <strong>de</strong> 20-29 ans, 34,8 mois chez celles <strong>de</strong> 30-39 ans <strong>et</strong> 38,6 mois chez celles <strong>de</strong> 40-49 ans. C<strong>et</strong>te corrélation entre l'âge <strong>de</strong>s mères <strong>et</strong> la longueur <strong>de</strong> l'intervalle intergénésique <strong>de</strong>vrait être nuancée dans la mesure où les femmes jeunes ayant souvent moins d’enfants sont moins enclines à différer l’arrivée d’une prochaine naissance 2 . Cela se confirme dans la répartition <strong>de</strong>s naissances selon la longueur <strong>de</strong> l’intervalle par rang <strong>de</strong> naissance. On observe en eff<strong>et</strong>, que l’intervalle augmente légèrement avec le rang <strong>de</strong> naissance, passant <strong>de</strong> 32,9 mois pour les enfants précédés d’un ou <strong>de</strong> <strong>de</strong>ux autres à 33,9 mois <strong>et</strong> 33,5 mois pour ceux qui sont précédés <strong>de</strong> plus d’enfants. Selon le sexe <strong>de</strong> l'enfant, on n'observe pas <strong>de</strong> différence significative <strong>de</strong> l'intervalle intergénésique. Par contre, les naissances qui suivent <strong>de</strong>s enfants décédés se produisent beaucoup plus rapi<strong>de</strong>ment que lorsque l'enfant précé<strong>de</strong>nt est toujours en vie (intervalle médian <strong>de</strong> 28,3 mois contre 34,0 mois). Par ailleurs, alors que seulement 5 % <strong>de</strong>s naissances surviennent dans les 18 mois lorsque l’enfant précé<strong>de</strong>nt est en vie, plus <strong>de</strong> 18 % <strong>de</strong>s naissances se produisent dans un délai inférieur à 18 mois lorsque l’enfant précé<strong>de</strong>nt est décédé. La durée <strong>de</strong>s intervalles intergénésiques diffère selon le milieu <strong>de</strong> rési<strong>de</strong>nce <strong>de</strong>s femmes. C'est en milieu rural qu'elle est la plus courte (33 mois contre 34,8 mois en milieu urbain). Par contre, la fréquence <strong>de</strong>s intervalles très courts (moins <strong>de</strong> 18 mois) semble plus élevée dans les villes qu’en zones rurales (8 % contre 6 %). Concernant les régions, on remarque <strong>de</strong>s écarts assez notables. L'intervalle intergénésique varie d'un maximum <strong>de</strong> 35,5 mois à Saint Louis <strong>et</strong> <strong>de</strong> 34,7 mois à Ziguinchor à un minimum <strong>de</strong> 31,7 mois dans celle <strong>de</strong> Kaolack. Les résultats montrent que le niveau d'instruction <strong>de</strong> la mère influence la durée <strong>de</strong> l'intervalle intergénésique : sa valeur médiane varie <strong>de</strong> 33,3 mois pour les naissances dont la mère a un niveau primaire à 37,6 mois pour celles dont la mère a un niveau d'instruction secondaire ou plus. Enfin, les résultats semblent m<strong>et</strong>tre en évi<strong>de</strong>nce une faible association positive entre le niveau <strong>de</strong> bien-être <strong>et</strong> la longueur <strong>de</strong> l’intervalle intergénésiques : celle-ci est <strong>de</strong> 36,2 mois le quatrième quintile <strong>et</strong> gar<strong>de</strong> à peu près la même valeur dans les autres quintiles (32,5 à 33,8 mois). 2 Seuls 0,5 % <strong>et</strong> 12.7 % <strong>de</strong>s femmes <strong>de</strong> 15-19 ans e <strong>de</strong> celles <strong>de</strong> 20-24 ans respectivement ont 3 enfants ou plus. 62 | Fécondité
